This agreement is a supplement and extension of your original contract with us.  

You must use up all the lessons purchased within one year from the date of signature. 

We kindly request you not to tip our trainers or give them any monetary rewards.  
We strongly encourage you to render written praises should you feel that your trainer is performing exceptionally well.

I have carefully read this Program Agreement, including the terms on the reverse page. I have received a copy of this agreement. I understand the terms and conditions and agree to be bound by them.
