可视 B 超在钳刮术中的应用

【摘要】目的： 探讨可视 B 超钳刮术较常规钳刮术所具有的优点。

方法： 我院 2007 年 12 月～2008 年 12 月收治的 48 例孕 11～14 周终

止妊娠患者随机分为两组，治疗组采用可视 B 超钳刮术，对照组采用

常规钳刮术，比较两组的手术时间、术中出血量、宫内残留物及二次

清宫情况。结果：对照组手术时间[(15.0±2.1) min]明显长于治疗组

[(8.0±1.8)min](P<0.05),出血量[(7.9±15.3)mL]明显多于治疗

组[(4.0±14.7)mL] (P<0.05)， 对照组 24 h 后复查 B 超 4 例(16.67%)

宫内有残留物，并行二次清宫术，治疗组 24 h 后复查 B 超无 1 例宫内

有残留物，两组差异有统计学意义(P<0.05)。结论：可视 B 超下钳

刮术整个手术过程由盲视操作变为可视操作，较常规钳刮术具有手术

时间短、出血量少、无残留、避免二次清宫的优点，值得临床推广。

[ABSTRACT] Objective: To investigate the advantages of

termination of pregnancy of 11 ～ 14 weeks througth forceps

curettage guided by video B mode sonography over conventional

forceps curettage. Methods: Forty eigth pregnant women going

to terminate pregnancy of 11 ～ 14 weeks from December 2007 to

December 2008 were randomized into two groups (group Aand B);

pregnant women of group A were undergone termination of

pregnancy through forceps curettage guided by video B mode

sonography, pregnant women of group B were undergone termination

of pregnancy through conventional forceps curettage. The

operative time, bleeding volume, residual of embryo, and

secondary curettage were compared between the two groups.

Results: The operative time of group A and B were (8.0 ± 1.8)

min and (15.0 ± 2.1) min respectively, and there was

significant difference between the two groups (P<0.05); the

amount of bleeding of group A and B were (7.9 ± 15.3) mL and

(40 ± 14.7) mL respectively, and there was significant

difference between the two groups (P <0.05); on the

ultrasound examination followed up in 24 h, 4 cases (16.67%) of

group B were found intrauterine residual of embryo, and they were

undergone second curettage, no residual of embryo were found in

group A, and there was significant difference between the two

groups (P<0.05). Conclusion: Termination of pregnancy of

11～14 weeks througth forceps curettage guided by video B mode

sonography has advantages over conventional forceps curettage;

because of visual operation, it becomes accurate and reliable,

with shorter operative time, less bleeding, no residue, and

avoidance of secondary curettage.

因避孕失败所致的非意愿妊娠和某种疾病或胎儿畸形，不宜

继续妊娠，可采用引流产方法终止妊娠, 根据孕周大小可选用不同终

止妊娠的方法[1，2]。孕 11～14 周胎儿已基本发育成形，此时羊水量

较少， 用吸宫术和中期引产来终止妊娠比较困难。 我院 2007 年 12 月～

2008 年 12 月采用可视 B 超下钳刮术对 24 例患者终止妊娠，获得满意

疗效，现报道如下。

1 临床资料

1.1 一般资料

本组孕 11～14 周患者 48 例，随机分为两组，治疗组 24 例，

年龄 18～40 岁，平均 29 岁，孕次£3 次;对照组 24 例，年龄 20～42

岁，平均 31 岁，孕次£3 次。48 例均为单胎妊娠，均无凝血系统功能

障碍及器质性疾病。两组年龄、孕产次等一般资料差异无统计学意义

(P﹥0.05)，具有可比性。

3
