摘 要：目的］
［ 报道板棒多轴螺钉系统后路枕颈固定的初步疗效，

探讨与其它内固定比较的优势。 ［方法］14 例接受颈枕 Summit 板棒多

轴螺钉系统固定治疗，男 9 例，女 5 例。齿突发育不良并 C1、2 脱位

3 例，椎管肿瘤 4 例，C1、2 转移癌并病理骨折、类风湿性关节炎并枕

颈不稳与颅底凹陷症各 2 例，寰椎爆裂骨折并寰枢寰枕不稳 1 例。12

例颈痛，13 例有不同程度的脊髓损害表现，JOA 评分平均 124 分。 ［结

果］所有螺钉位置良好，无螺钉植入相关并发症。所有颈痛缓解。13

例获随访，平均 14 个月。1 例颈枕未融合，枕骨螺钉拔出，其它固定

满意并坚固融合。神经功能均有不同程度恢复，JOA 评分改善率为

630％。 论］板棒多轴螺钉系统后路枕颈固定安全可靠，取得了满 ［结

意的临床疗效。与目前常用的固定方法比较具有一定优势。

Abstract： Objective］To report the initial outcomes of ［

occipitocervical fixation using platerodpolyaxial screw system

and investigate whether this new system offers any advantage

over other existing methods of fixation［Method］Fourteen cases

（ male 9,female 5 ） accepted cervicooccipital fixation with

platerodpolyaxial screw systemThere were 3 cases of odontoid

dysplasia with dislocation;spinal canal tumor in 3;2 cases of

metastatic carcinoma with pathologic fracture of C1 and

C2,rheumatoid arthritis with cervicooccipital instability,and

fasilar impression;burst fracture of C1 with

枕颈后路固定方法很多，如骨板加钢丝、棒加钢丝、枕颈钢板，

这些内固定都存在不同的缺点〔1、2〕 为克服上述内固定的不足，很 。

多新的枕颈内固定器械已问世，Summit 板棒多轴螺钉枕颈固定系统是

其中一种，其临床疗效仍不清楚，也未见专门报道。作者应用该系统

行枕颈固定 14 例，初步临床效果满意，总结如下。

1 材料与方法

11 一般资料

2003 年 9 月~2005 年 3 月，14 例患者接受枕颈 Summit 板棒多轴

螺钉系统固定治疗。男 9 例，女 5 例；平均年龄 483 岁。齿突发育不

良并 C1、2 脱位 3 例，椎管肿瘤 4 例，C1、2 转移癌并病理骨折、类

风湿性关节炎并枕颈不稳与颅底凹陷症各 2 例，寰椎爆裂骨折并寰枢

寰枕不稳 1 例。12 例颈痛，13 例有不同程度的脊髓损害表现，如四肢

无力、行走不稳、手活动不灵、肌张力增高、腱反射亢进及病理反射

等。JOA 评分为 10~15 分，平均 124 分。所有患者术前行 X 线片及 MRI

检查，CT 扫描和三维重建 8 例，5 例行 MRA 检查。
