化学法与免疫胶体金法检测粪潜血的比较

[摘 要] 目的:探讨粪潜血二种检测方法的优劣和应用。方法:用邻联甲苯胺和便隐血胶体金检测试纸分别检测200例粪标本进行对照实验并分析。结果:二者一致性符合率为90%。结论:化学法灵敏度、特异性较胶体金法低,受饮食影响因素大,胶体金法有假阴性,故建议二者结合判断。

The Comparison Between the Chemical Method and Immunogold Method in Detecting Sedoccult Blood
Abstract:Objective To explore the comparability between the chemical method and immunogold method in detecting sed occult blood.Methods To detect the sed occult blood of 200 specimens with both orthotolidine method and immunogold method.then analyse the results.Results The coincidence of the detecting results between the two methods is 90%.Conclusion Food and drink impact the results of chemical method very much.so chemical method is lesssensitive and specific than immunogold method.But immunogold method also has false positivereaction,therefore we suggest detecting sed occult blood with the both two methods.

粪便潜血试验(fecal occult biood test)是诊断和监测消化道出血性疾病的有效常规检查,也是发现消化道肿瘤的重要手段之一。目前最常用的方法有化学法(包括邻联甲苯胺法、潜血粉法、愈创木脂法)和免疫学方法(胶体金标记夹心免疫法)。传统化学法成本低,但假阳性多、灵敏度低,对操作人员有致癌性;新兴的免疫胶体金法成本略高,但因其特异性好而越来越广泛使用,本文仅以邻联甲苯胺法与免疫胶体金法相比较,探讨二者在临床使用上的价值。
1 材料与方法
1.1 试剂
便隐血胶体金检测试纸,北京万华普曼生物工程有限公司提供,批号20050724;3%H2O2自配,邻联甲苯胺由上海远航试剂厂生产,冰乙酸及无水乙醇由广州东红化工厂生 产,均为AR级。称取邻联甲苯胺1 g溶于冰乙酸及无水乙醇各50 ml中,棕色瓶保存(4 ℃保存8周~12周)。
1.2 标本
本院200份无重复住院患者新鲜大便,均为成年人,女89例,非月经期,男111例(均排除痔肛裂出血)。
1.3 方法
每份标本均做邻联甲苯胺法和便隐血胶体金试纸检测、邻联甲苯胺法按全国操作规程,便隐血胶体金试纸法按说明书操作。
