二、监理工作范围
负责本项目施工阶段的监理工作,包括本工程的土建部份、给排水、电气、小区道路等范围。
三、监理工作内容
包括:工程质量控制、进度控制、投资控制、合同管理、组织协调、安全文明施工监理等工作。
具体内容主要有:
1)项目监理机构在项目总监指导下,编制本工程的“监理规划”和各专业“监理实施细则”,作为本工程监理的指导性和操作性文件;
2)组织设计技术交底和图纸会审;
3)审批工程开工报告,检查施工准备情况,对施工单位及选择的分包单位进行资格审查;
4)审核施工单位提交的施工组织设计、施工技术方案,并检查其落实情况;
5)审查施工单位提出的工程总进度计划、年度及月度施工计划和工程进度报告。
6)审查施工单位提出的工程项目各阶段及各年、季、月度资金使用计划,并控制其执行。审核施工单位申报的工程款与签证,并审核工程结算。
7)采用旁站、巡视、平行检查的手段,监督施工单位严格按照工程技术标准、施工规范及操作规程施工。
8)审核施工单位提出的材料、构配件及设备的采购计划,检查其材料的型号、规格等是否符合质量标准,并严格检查主要材料、构配件及设备的出厂合格证明书及材质报告,对进场的材料进行见证取样送检,设备进行检测,送检及检测合格后方允许其使用,对不合格的材料及设备要求施工单位立即清理退场。
9)隐蔽验收和分部分项验收。
10)处理工程质量事故。
11)控制现场的安全文明施工。
12)负责各施工单位、建设单位之间的协调工作,主持工程例会。
13)组织竣工初验,审核施工单位的竣工验收资料,协助业主进行竣工验收、备案工作。
14)负责保修期的协调及质量事故处理工作。
四、监理工作目标
1、工期目标:合同期内完成全部工程施工内容。
2、质量目标:本工程的质量控制目标为合格。
3、投资目标:将本工程的投资控制在施工合同的范围内,并尽量节约投资。
4、安全目标:要求满足河源职业技术学院的安全体系要求,将本工程的安全质量事故控制在最低限度内。

根据惠市规建函[2006]379号文件《惠州市工程项目监理人员配置表》的有关要求进行配置,现场监理人员由总监理工程师、总监理工程师代表、各专业监理工程师(土建、给排水、电气、安全文明施工)、监理员等人员组成,详见“项目监理组织机构图”和“项目监理人员一览表”。

(二)、人员进场计划
自开工至工程竣工期间,总监、总监代表将不定期去工地进行巡视监理,土建工程师将驻工地现场进行全过程监理,水、电、空调工程师自基础及结构主体施工阶段进行巡视监理,在进入本专业施工高峰期间将驻现场进行监理,直至本专业竣工验收。
三、项目监理机构的人员岗位职责
(一)、总监理工程师
1、确定项目监理机构人员的分工和岗位职责;
2、主持编写项目监理规划、审批项目监理实施细则,并负责管理项目监理机构的日常工作;
3、审查分包单位的资质,并提出审查意见;
4、检查和监督监理人员的工作,根据工程项目的进展情况可进行人员调配,对不称职的人员应调换其工作;
5、主持监理工作会议,签发项目监理机构的文件和指令;
6、审定承包单位提交的开工报告、施工组织设计、技术方案、进度计划;
7、审核签署承包单位的申请、工程款支付证书和竣工结算;
8、审查和处理工程变更;
9、主持或参与工程质量事故的调查;
10、调解建设单位与承包单位合同争议、处理索赔、审批工程延期;
11、组织编写并签发监理月报、监理工作阶段报告、专题报告和项目监理工作总结;
12、审核签认部分工程和单位工程的质量检验评定资料,审查承包单位的竣工申请,组织监理人员对待验收的工程项目进行质量检查,参与工程项目的竣工验收;
13、主持整理工程项目的监理资料。
(二)、总监理工程师代表
1、负责总监理工程师指定或交办的监理工作;
2、按总监理工程师的授权,行使总监理工程师的部分职责和权利。
3、总监理工程师不在场时负责现场的协调工作。
4、负责本专业的专业监理工程师工作。
(三)、专业监理工程师
1、负责编制本专业的监理实施细则;
2、负责本专业监理工作的具体实施;
3、组织、指导、检查和监督本专业监理员的工作,当人员需要调整时,向总监工程师提出建议;
4、审查承包单位提交的涉及本专业的计划、方案、申请、变更,并向总监理工程师提出报告;
5、负责本专业分项工程验收及隐蔽工程验收;
6、定期向总监理工程师提交本专业监理工作实施情况报告,对重大问题及时向总监理工程师汇报和请示;
7、根据本专业监理工作实施情况做好监理日记;
8、负责本专业监理资料的收集、汇总及整理,参与编写监理月报;
9、检查进场材料、设备、构配件的原始凭证、检测报告等质量证明文件及其质量情况,根据实际情况认为有必要时对进场材料、设备、构配件进行平行检验,合格时予以签认;
10、负责本专业的工程计量工作,审核工程计量的数据和原始凭证。
(四)、监理员
1、在专业监理工程师的指导下开展现场监理工作;
2、检查承包单位投入工程项目的人力、材料、主要设备及其使用、运行状况,并做好检查记录;
3、复核或从施工现场直接获取工程计量的有关数据并签署原始凭证;
4、按设计图及有关标准,对承包单位的工艺过程或施工工序进行检查和记录,对加工制作及工序施工质量检查结果进行记录;
5、担任旁站工作,发现问题及时指出并向专业监理工程师报告;
6、做好监理日记和有关的监理记录。
第三章 监理工作程序
一、监理工作流程
(一)成立项目监理机构
公司法人下达总监任命书,公司经营部将项目资料移交监理中心,监理中心协同总监组成项目监理机构进场正式开展工作,办公室负责后勤支持。
(二)收集监理资料,熟悉现场情况,编写监理规划、监理细则
1、监理资料
项目监理机构收集以下资料:
1) 施工投标书及相关文件、合同
2) 设计图纸,工程勘察报告
3) 政府相关的批文
4) 相关法律、法规、规范
2、熟悉现场情况
1) 场地情况:场地“五通一平”;周边建筑物;场地土质;场地地下管线;周边道路管制规定;
2) 人员情况:建设单位人员;施工单位人员及各相关单位的人员组织;
3) 资料情况:图纸;合同;技术规范;政策;法规;
4) 规划局测量放线点。
3、编写监理规划
总监主持编制,专业监理工程师参加。经总监、公司副总工程师(总工办主任)、总工程师签署后报建设单位。
4、编写监理实施细则
由各专业监理工程师负责编写本专业监理实施细则。经总监理工程师批准后实施。对重大及技术含量高的项目,监理细则须经公司技术部门批准方可实施。监理细则应符合监理规划的要求,并应结合工程项目的特点,做到详细具体,具有可操作性。监理实施细则包括以下主要内容:
1) 专业工程的特点;
2) 监理工作的流程;
3) 监理工作的控制要点依据及目标值;
4) 监理工作的方法及措施。
(三)召开第一次工地会议
建设单位主持召开第一次工地会议。建立施工、监理、设计、建设单位之间的沟通渠道。各方项目负责人(合同指定的人选)必须参加。会议包括以下内容:
1、建设单位、施工单位和监理单位分别介绍各自驻场组织机构、人员及其分工;
2、建设单位介绍工程开工准备情况(主要是场地三通一平、报建手续、外部环境、设计图纸到位等);
3、建设单位根据委托监理合同宣布对项目总监理工程师的授权;
4、施工单位介绍对本工程施工准备情况(人员、设备、材料、临时设施、技术等);
5、建设单位和总监理工程师对施工准备情况提出相关意见和要求;
6、总监理工程师介绍监理规划和有关监理工作程序的主要内容;
7、确定各方在施工过程中参加工程例会的主要人员,以及工程例会召开的周期、地点及主要议题;
8、各方签署文件权限声明;
9、文件收发人。
(四)开工前的若干工作
1、核对建设单位需提供的条件:
1)合同签订情况;
2)“五通一平”——泛指通水、通电、通道路、通通讯、通煤气,场地平整等相关施工条件;
3)资金到位情况;
4) 施工许可证;
5) 图纸是否已满足施工需要;
6) 规划局放线、放线册;
7) 建设单位针对本项目的组织情况;
2、图纸会审
由建设单位或监理公司主持,施工单位记录整理。
3、对施工单位的审核
1) 对其资质的认定
2) 组织机构及质量管理体系
3) 施工组织设计(专项施工方案)
4) 人员到位情况
5) 施工设备及原材料到位情况
(五)开工
施工单位在具备开工条件后,提出开工申请,填写相应表格报监理公司,总监签署意见后报建设单位,建设单位同意后由总监签署开工令,开始计算工期。
(六)施工过程控制
1、对材料的控制
1) 材料供应
合同中指定的品牌按合同采购;合同中未指定的,由施工单位提前提供若干符合设计要求的产品报建设单位选择;
2) 材料检验
对进场材料,施工单位除了需提供出厂的合格证明文件外,还需在监理的现场监督下按规定比例送检。检测单位按政府规定选取,须有相应资质。送检时由我公司持有“见证员证”的监理人员陪同见证。检验结果出来后,施工单位须出示原件并提供监理一份复印件备案。对检验不合格的产品,监理有权要求施工单位清理退场。
2、施工工序的控制
1) 对组织及施工人员的控制
管理员及其效率;技术人员的到位情况;人员的持证上岗,技术熟练程度;施工交底情况。
2) 施工方案
施工单位除了总体施工方案外,还必须在施工之前编制专项施工方案,专业监理工程师审核后由总监审定批准执行。
3) 旁站及巡视监理
对重要及重点部位以及隐蔽项目,监理将派员旁站监理,监督施工全过程。其它情况采用巡视的办法,定时不定时现场巡查。
4) 停止施工的命令
可能危及生命安全及重大财产损失时,或者施工质量严重失当经警告施工单位拒不改正时,监理将发出停工指令。除非紧急情况,监理将事先询问建设单位意见再发出停工令。隐患消除后,施工单位填写表格申请复工,监理检查无误后报建设单位,建设单位同意后下达复工指令。停工令及复工令必须由总监签发。
3、分部分项验收
1) 分项的验收
施工单位自检合格后提出申请,监理知会建设单位参加分项验收。建设单位不参加时,由监理直接验收。分项验收由总监或总监指定的专业工程师负责并签署文件。重要分项的验收应知会设计单位与质监站,由其决定是否派员参加。验收不合格的项目责令施工单位返工修补或重做。
2) 分部验收
由总监组织施工、设计、建设等单位共同验收,质监站派员参加,重要分部施工单位须事先按质监站的规定申报,质监站批准后方可验收。
