申 请 企 业 须 知

1、申请企业必须严格按照《消防类产品型式认可实施规则》中工厂基本条件的要求建立并实施有效的质量管理，确保企业的质量保证能力符合实施规则的基本要求；
2、申请企业必须符合《消防类产品型式认可实施规则》中生产设备及检验设备的最低配置要求；
3、申请企业须严格遵守型式认可程序及时限要求，正常情况下，接到工厂检查通知后的30日内应接受工厂条件检查，并应在规定时间内完成对不符合项的整改工作，自抽样之日起15日内将样品送到指定检测机构进行检验，并按期缴纳相关费用；如遇特殊原因，必须及时通报公安部消防产品合格评定中心；
4、申请企业获证后，必须严格按照《消防类产品型式认可实施规则》的规定，正确使用型式认可证书和标志。对有违规者，公安部消防产品合格评定中心将进行严肃处理。
5、申请企业获证后，所生产及销售的产品必须与发证检验样品保持一致，产品质量必须符合型式认可规则和相关标准要求。若必须对涉及产品一致性的关键因素，如关键原器件、原材料、关键设计及主要生产工艺发生变更，以及生产场地搬迁、法人及主要技术人员发生变更时，获证企业必须及时向公安部消防产品合格评定中心申报，由中心按照《消防类产品型式认可实施规则》要求安排确认工作。未经中心确认，不得生产及销售产品，对违规者，公安部消防产品合格评定中心将进行严肃处理，由此而导致的一切责任均由违规企业承担。
The Notice of the Applicant

1. The Applicant must not only constitute a quality control and bring into effect which according to the request of the factory’s postulate, but also make sure the quality guarantee capacity of the corporation according to the request of the implement rules.
2. The applicant must measure up the lowest configuration requirement of producing equipment and checking equipment that refers to the 《The Implement Rules Of The fire fighting product Type-Approval》.
3. The applicant must follow the authentication procedure and the time limit requirement. In normal condition, the enterprise should accept the work condition inspection within 30 days after receive the information, and finish the modification of the unaccepted item within stated time. The enterprise should send the sample to the appointed examine institution in 15days after the inspection, and pay for the related bill. The enterprise should declare to CCCF in time if there is any special case.
4. After getting the certificate, the applicant must follow the implement rules strictly and use authorized certificate and symbol correctly. CCCF will sternly punish the foul.
5. After getting the certificate, the applicant must keep the products they produce and sell the same with the inspecting samples. The quality of the products must measure up the authentication regulations and the correlative standards. If the some key factors that related to the coherence of the products have to be modified, such as key source apparatus, raw material, core design and primary produce technique, as well as the workplace, juridical person and key technical personnel change, the enterprise must declare to CCCF. And CCCF validates the situation according to the implement rules. The enterprise can't produce and sell any products without the validation. CCCF will sternly punish the foul. And the foul is charged of all the correlative obligation.

1．本企业遵循“诚信第一”的基本原则，承诺遵守国家有关法律法规及公安部消防产品合格评定中心（以下简称CCCF）制定的型式认可实施规则的所有要求及型式认可程序。
2．本企业同意按型式认可规则的要求提供评价所需文件和资料，所提供的信息均真实有效，否则承担由此引起的一切法律责任。

5、本企业完全接受《申请企业须知》中所涉及的全部内容。
Statement
I, on the behalf of the Applicant, make the statement hereby:
1. The Applicant is willing to comply with the Implement Rules for Type-Approval formulation which by China Certification Center for Fire Products (So-called as CCCF).
2. The Applicant does agree to submit the necessary documents and information which truthfulness has been ensured by the Applicant, From this otherwise undertakes all legal liability which causes.
3. The Applicant guarantee to make the payment in accordance with the Type-Approval Contract and/or the relevant requirement.
4. The Applicant will keeping the relevant information about authentication properly, such as the prove report, jerque report of factory and the validate of the authentication change, which will be used in the supervise inspect, sudden inspect and the supervisal task after application. CCCF will not toe the scratch which must be beared by the factory or merchant who get the product eligibility authentication.
5. The Applicant does accept the contents 《The notice of the Applicant》contained.

三、申 请 企 业 承 诺

我代表申请企业郑重承诺：
1．始终遵守型式认可计划安排的有关规定；
2．为进行评价作出必要的安排，包括配合检查组的工作、允许检查组进入需进行检查的区域、查阅所有的记录（包括内部审核报告、人员评价文件等）、提供必要的交通工具等；
3．按合同规定和相关规定，及时缴纳各项费用，及时从“中国消防产品信息网”获取相关信息；
4．按有关法律法规、规章的要求，正确使用证书和标志；
5．谨慎使用产品型式认可结果，不损害公安部消防产品合格评定中心的声誉；不对社会产生误导，不未经许可发表声明；
6．当证书被暂停、撤销、中止型式认可程序时，立即停止涉及型式认可内容的广告及宣传，并按要求将所有型式认可文件（至少应包含型式认可证书及型式检验报告）交回公安部消防产品合格评定中心，封存型式认可标志；
7．在传播媒体中对产品型式认可内容的引用，严格遵守国家的相关要求。

合同管理制度

1 范围
本标准规定了龙腾公司合同管理工作的管理机构、职责、合同的授权委托、洽谈、承办、会签、订阅、履行和变更、终止及争议处理和合同管理的处罚、奖励；
本标准适用于龙腾公司项目建设期间的各类合同管理工作，厂内各类合同的管理，厂内所属各具法人资格的部门，参照本标准执行。

4 职责
4.1 总经理：龙腾公司经营管理的法定代表人。负责对厂内各类合同管理工作实行统一领导。以法人代表名义或授权委托他人签订各类合法合同，并对电厂负责。
4.2 工程部：是发电厂建设施工安装等工程合同签订管理部门；负责签订管理基建、安装、人工技术的工程合同。
4.3 经营部：是合同签订管理部门，负责管理设备、材料、物资的订购合同。
4.5 合同管理部门履行以下职责：
4.5.1 建立健全合同管理办法并逐步完善规范；
4.5.2 参与合同的洽谈、起草、审查、签约、变更、解除以及合同的签证、公证、调解、诉讼等活动，全程跟踪和检查合同的履行质量；
4.5.3 审查、登记合同对方单位代表资格及单位资质，包括营业执照、经营范围、技术装备、信誉、越区域经营许可等证件及履约能力（必要时要求对方提供担保），检查合同的履行情况；
4.5.4 保管法人代表授权委托书、合同专用章，并按编号归口使用；
4.5.5 建立合同管理台帐，对合同文本资料进行编号统计管理；
4.5.6 组织对法规、制度的学习和贯彻执行，定期向有关领导和部门报告工作；
4.5.7 在总经理领导下，做好合同管理的其他工作，
4.6 工程技术部：专职合同管理员及材料、燃料供应部兼职合同管理员履行以下职责：
4.6.1 在主任领导下，做好本部门负责的各项合同的管理工作，负责保管“法人授权委托书”；
4.6.2 签订合同时，检查对方的有关证件，对合同文本内容依照法规进行检查，检查合同标的数量、金额、日期、地点、质量要求、安全责任、违约责任是否明确，并提出补充及修改意见。重大问题应及时向有关领导报告，提出解决方案；
4.6.3 对专业对口的合同统一编号、登记、建立台帐，分类整理归档。对合同承办部门提供相关法规咨询和日常协作服务工作；
4.6.4 工程技术部专职合同管理员负责收集整理各类合同，建立合同统计台帐，并负责
