篇一：全民健康生活方式行动示范社区创建工作计划
宾县宾州镇中心街社区全民健康生活
方式行动创建工作计划
根据《全民健康生活方式行动总体方案（2007-2015
年）》，为推动宾州镇中心街创建全民健康生活方式行动示范村，特制定本计划。
一、创建目的
通过全民健康生活方式行动示范创建活动，营造健康生活方式支持环境，普及健康生活方式相关知识，提供健康生活方式行为指导，培养民众健康意识和健康行为能力，最终提高居民健康水平。
二、创建工作基本内容
1. 环境建设。
（1）环境整洁。
（2）有固定的宣传栏、橱窗等健康教育窗口。
（3）有健康教育活动室。
2．活动开展。
（1）每年开展3种以上居民广泛参与的健康生活方式
活动，如知识竞赛、健康膳食设计比赛、健身比赛、健康家庭评选等。
（2）每年为每户居民发放1种以上健康生活方式宣传
材料。
（3）每年开展4次以上健康生活方式相关知识讲座。
（4）利用固定宣传栏或橱窗开展“健康一二一”知识
宣传，定期更换宣传内容。
（5）健康教育活动室提供5种以上可供群众取阅的健
康生活方式宣传材料，有身高体重计、腰围尺、壁挂BMI尺、膳食宝塔挂图等设施，有控油、限盐等支持工具展示。
（6）社区内有3个以上群众性健身活动团体。
（7）开展居民广泛参与的健康生活方式活动，如知识
竞赛、健康膳食设计比赛、健身比赛、健康家庭评选等。

董浜镇市镇社区2010年建设健康社区工作计划
时间：2010-05
一、指导思想
坚持以“邓小平理论”和“三个代表”重要思想为指导，深入贯彻落实科学发展观，以提高社区居民健康综合素质为切入点，积极倡导健康文明的生活方式，不断提高居民群众健康意识和自我保健能力，积极推进健康社区建设。
二、二个目标
1、总体目标
在“建设健康社区三年行动计划”中，面上求进，点上突破，切实做好2010年的各项工作，促进社区“人与人、人与自然、人与社会”的和谐发展。
2、具体目标
重点抓好“五个人人”项目，开展“人人知道自己的血压”、“人人参加健身运动”、“人人养成健康行为”、“人人掌握自救互救技能”、“人人参与无偿献血”活动，促进建设健康社区有序推进。
三、三个重点
1、高血压防治
与社区卫生服务中心以及有关部门联手，开展健康教育咨询义诊活动，在社区群众中普及高血压防治知识，组织开展老年人健康体检活动，健康档案率达80%以上。
2、人人运动
与老年体协联手开展“人人参加健身运动”的群体性体育活动，组织开展好“迎世博、展风采”第十五次“全民健身月”活动，把开展群体性健身运动作为健康进社区的基本点，逐步建成点、线、面的群众体育健身网络。以组织开展健身活动成为居民日常生活的一项规律，加强自身锻炼。
3、巩固绿色社区创建成果
巩固常熟市绿色社区创建成果，争创苏州市绿色社区，是今年的重点实事项目，社区加大力度对社区的环境绿化、美化。使社区绿化覆盖率达40%，营造绿色健康、安全文明、卫生清洁的社区环境。
四、四个内容
1、创建健康小区2个（徐家湾、永安新村），健康单位示范点5个，健康家庭示范户 50 户，健康促进志愿者60名。发动健康小区、健康单位共同参与健康社区工作，充分发挥健康志愿者、健康单位的自身特点，有选择性地开展各项活动。
2、抓好“五个人人活动”的有序开展。开展“人人知道自己血压”、“人人参加健身运动”、“人人参与无偿献血”活动，加强无偿献血的知识宣传，发动居民自愿参加无偿献血。开展“人人掌握自救互救技能”培训活动，提高居民自救互救意识和技能，掌握基本的救护本领。开展“人人养成健康行为”活动，发动居民群众开展健康行为大讨论，对身边不健康的卫生陋习进行评议，大力弘扬文明、健康的良好风尚。
3、积极开展“清洁家园”活动，精心组织，广泛发动居民参与整治居民小区和公共场所环境卫生的公益性活动，美化社区环境。对中小道路、
农贸集市等，与社区居民群众密切相关的环境进行重点整治，取得居民群众的认可，提高满意率。对照标准，查找差距，对薄弱环节加强整改，巩固国家卫生镇创建成果。
4、抓好季节性生物虫害控制工作，加强对居民区的环境整治，重点对垃圾箱房、绿化地等四害孳生地进行管理，清除孳生源，降低四害密度，达到国家卫生镇要求，同时要做好居民对除四害工作的专业指导。
五、五项措施
1、加强领导，落实任务
将建设健康社区工作纳入社区工作的议事日程，加强组织领导，建立建设健康社区工作领导小组，完善工作机制，健全组织网络。按照实施计划的目标任务和重点项目组织落实，并进行目标细化分解，责任到人，把建设健康社区工作作为社区居委会的一把手工程，由主要领导负责、分管领导具体负责，确保完成各项目标任务。
2、宣传发动，统一思想
2010年是我社区“建设健康社区三年行动计划”的开局之年，要通过各种有效的宣传方式和信息平台大力宣传建设健康社区的重要性和长远意义，不断提高社区居民健康知识的知晓率和参与率，三年内居民的健康知识知晓率达70%以上。大力营造良好的生活环境，改变不良生活习惯，使居民健康行为形成率在80%以上。
3、整合力量，全民参与
建设健康社区工作是一项系统工程，其内容涉及卫生、城管、文教、计生多个部门，必须有效整合力量，形成工作上的互动，得到各相关部门
和广大居民的大力支持和配合。广泛动员和引导社会团队等非政府组织积极参与创建活动，充分发挥志愿者队伍作用，形成政府倡导、部门运作、社会参与的良性机制。
4、制度先行，有序推进
建立和完善建设健康社区的各项工作制度。一是例会制度。定期召开健康社区、健康小区、健康单位工作例会，抓进度，抓落实，沟通信息，交流经验，取长补短；二是目标责任制度。按照目标分解，逐一明确各相关部门职责；三是检查督查制度。对建设健康社区的工作进展情况定期进行检查和自查，确保每一个环节落实到位。
5、强化管理，探索创新
在建设健康社区的工作中，加强管理力度，做到建设有规划，任务有落实，管理出成效。同时结合社区自身实际，借鉴其他社区建设的先进经验，努力实践，不断探索，拟订能体现董浜市镇社区特色的项目，争取出成绩，出经验。

东风社区2012年健康教育工作总结
加快创建国家卫生城市步伐，增强市民的卫生意识和文明素质，努力塑造文明、祥和、繁荣的社区新形象，今年我社区健康教育工作在上级有关部门的指导下，紧密围绕创建国家卫生城目标，积极开展各项工作。现将一年的工作情况总结如下：
一、领导重视，组织机构健全
首先成立健康领导小组，社区主任亲自抓，并把这项工作放到重要位置抓紧抓好，对如何搞好我社区的健康教育工作进行认真研究。要求社区抓好健康教育。首先，配合创建文明卫生城，积极开展全民性普及健康卫生知识和除害灭病知识教育，提高群众自我保健能力。社区居委会对这项工作十分重视，做到有人抓有人管。由于各级领导重视，机构人员落实，从而保证了我社区健康教育工作的顺利开展。
二、围绕创建国家卫生城，大力开展健康教育工作
社区充分利用各种宣传工具广泛宣传灭鼠灭蟑知识，使广大群众充分认识灭鼠灭蟑的重要性，达标与改造环境，增进健康，促进经济建设的关系，提高居民的意识，从而自觉投入灭鼠灭蟑达标活动。经多次对辖区居民住宅区、公共场所、水沟、厕所用药物消杀，有效地控制生长，为创建国家
卫生城打下良好基础。有效地防止了各种传染病的发生，确保了人民群众的健康。
三、各方配合，认真抓好社区健康教育。
社区组织给社区居民讲授个人卫生和家庭卫生知识。通过爱国卫生运动开展健康教育爱国卫生运动是一项群众运动，因而爱卫会在创建国家卫生城的各项工作中，把健康教育、宣传发动作为一个重要内容来抓，从而促进各项工作的开展。在今年的爱国卫生月活动中，利用报刊、黑板报等各种宣传工具，宣传搞好爱国卫生的重要性，向广大群众宣传卫生防病和个人卫生，使其养成良好的卫生习惯。社区开展各种形势宣传基本做到家喻户晓，人人皆知。
四、人人参与，使健康教育得到广泛开展。
我社区和驻地单位组织健康教育讲座次免费测量血压 次 合唱队、腰鼓队、秧歌队、舞蹈队在用文艺表演的形式宣传卫生健康知识。
总之，一年来我社区的健康教育在各级领导重视和支持下开展了一些工作，为提高我社区居民的卫生意识和卫生素质起到一定作用，收到一定效果。我们决心，在新的一年里，在区政府、街办的领导下，进一步抓好我社区的健康教育工作，为提高我区广大群众的文明卫生素质，创建国家卫生城而努力工作。

东风社区2012年健康教育工作计划
为了更好地贯彻落实三桥街道健康教育工作要点精神，进一步完善社区健康教育与健康促进工作体系，组织开展多种形式的健康教育与健康促进活动，广泛普及重大传染病和常见、多发慢性非传染性疾病防控知识，进一步提高社区居民群众健康知识水平和自我保健能力。根据社区实际情况，制定了社区2010年健康教育工作计划，内容如下：
一、 健全组织机构，完善健教工作网络
完善的健康教育网络是开展健康教育工作的组织保证和有效措施，今年我们将结合本社区实际情况，调整充实健康教育领导小组，进一步健全健康教育组织机构；加强健康教育管理基础工作，进一步完善健康教育资料，将健康教育工作列入社区工作计划，加强各类人员健康教育；进一步建立健全集预防、保健、健康教育、计划生育工作计划等为一体的社区卫生体系，把健康教育工作真正落到实处。
二、 突出防病重点，开展健教活动
开展预防控制艾滋病、结核、人感染高致病性禽流感、乙型病毒性肝炎等重大传染的健康教育与健康促进工作；同时广泛普及防治心脑血管疾病、恶性肿瘤、糖尿病等慢性非传染性疾病的卫生科普知识，积极倡导健康文明的生活方式，促进人们养成良好的卫生行为习惯。结合实际，制定应
对突发公共卫生事件健康教育、健康促进工作预案与实施计划，对公众开展预防和应对突发公共卫生事件知识的宣传教育和行为干预，增强公众对突发公共卫生事件的防范意识和应对能力。
三、普及科学健康知识，提高居民群众“两率”
一是利用本社区健康教育基地，采取群众喜闻乐见的健教方式，开展一些寓教于乐的健康教育活动。健康知识宣传和普及，正确引导社区居民积极参与各项有益身心健康的活动，引导居民把被动的“为疾病花钱”转变为主动的“为健康投资”，从根本上提高居民自身的健康知识水平和保健能力。
二是充分发挥社区的标语、专栏、板报等宣传阵地，宣传卫生常识，普及与健康相关知识。
三是抓好社区健身队伍的健身活动。利用活动室等健身场地，定期开展老年健身、棋牌赛等活动，组织开展秧歌表演、健身晨练活动，丰富居民的业余文化生活。
四是对居民广泛开展控烟教育，做到办公室、会议室有明显的禁烟标志，努力落实禁烟制度。
五是以老年人、妇女、青少年、流动人口四种人群为重点，广泛开展老年保健、老年病防治与康复等多种形式的健康教育和健康促进活动，免费为老年人测量血压和健康咨
询；做好计划生育工作，提倡晚婚晚育、少生、优生、优育，提高人口素质，组织妇女病体检，为她们提供优质服务，保护妇女的合法权益；做好青少年的健康教育，道德教育、心理健康教育、青春期卫生保健教育等活动。
同时把重点人群教育与普及教育有机
