外源性清蛋白输入对 ARDS 小鼠 CRP 和前降钙
素影响
【摘要】 目的 了解急性呼吸窘迫综合征(ARDS)小鼠液体复苏
时给予外源性清蛋白对 C 反应蛋白(CRP) 前降钙素(PCT)以及肺湿 、
质量、干质量的影响。方法 将 108 只昆明小鼠随机分为两组,正常对
照组 12 例,腹腔注射 15 mg/kg 生理盐水;致伤组 96 例,腹腔注射内
毒素(LPS)15 mg/kg,制备 ARDS 模型。致伤组分为未治疗组和治疗
组,治疗组又分为低浓度清蛋白组(尾静脉注射 0.5 g/L 人血清清蛋
白 10 mL/g 体质量) 高浓度清蛋白组(尾静脉注射 50.0 g/L 人血清 、
清蛋白 10 mL/g 体质量) 羟乙基淀粉组(尾静脉注射 60.0 g/L 羟乙 、
基淀粉 10 mL/g 体质量) 。正常对照组于注射后 4 h 断颈取血,致伤
后各组分别于注射后 4、8、16、24 h 断颈取血(每次 6 只小鼠) ,用
ELASA 法测定血清 CRP 和 PCT; 同时留取肺测湿质量、 干质量。 结果 PCT
在注射后 24 h 内随时间延长而升高, 异有显著性 H=17.97~21.60,
差 (
q=4.08~6.88,P<0.01) 与未治疗组比较,各治疗组 PCT 8、16、24 。
h 均下降(H=26.43~27.44,q=4.08~6.27,P<0.01) 。高浓度清蛋
白组与羟乙基淀粉组差异无显著性(P>0.05) 与未治疗组比较,低 。
浓度清蛋白组 CRP 下降(t=23.37、28.32,H=15.16、19.55, q=4.08、
4.58,P<0.01) ;高浓度清蛋白组与羟乙基淀粉组 CRP 24 h 无明显差
异(P>0.05) 。致伤组小鼠肺湿、干质量比值 4 h 上升,8 h 达到高
峰,16 h 有所下降,24 h 稍有恢复,与正常对照组比较,差异有显著
意义 H=23.15~24.70,
( q=4.28~6.87, <0.01) 结论 输入 50.0 g/L
P 。

人血清清蛋白能降低早期 ARDS 肺含水量,降低 CRP 和 PCT,减轻炎症
反应,有助于早期 ARDS 的治疗。

[ ABSTRACT ] ObjectiveTo investigate the effects of
exogenous albumin on C-reactive protein CRP) and procalcitonin (
(PCT) and the lung wet /dry (W/D) weight in mice with acute
respiratory distress syndrome ( ARDS ) . Methods This study
consisted of 108 Kunming mice, which were randomized to two
groups: normal control, 12 mice, 15 mg/kg of normal saline and
vulnerated groups(96 mice, 15 mg/kg of Endotoxin (LPS) to
prepare ARDS models, both normal saline and LPS were in injected
peritoneally. Treatment groups were divided into
low-concentration-albumin (LCA) group (0.5 g/L of human serum
albumin10 mL/g body mass), high-concentration-albumin (HCA)
group (50.0 g/L of human serum albumin 10 mL/g body mass),
and hydroxyethyl-starch (HS) group (60.0 g/L hydroxyethyl
starch 10 mL/g body mass), the medicines were given through
vena caudalis in the three groups. Blood samples were collected
at 4 h after injection by breaking the neck in normal control,
and at after4 h, 8 h, 16 h,and 24 h in vulnerated group (six
mice per each time). Serum CRP and PCT were determined by ELASA,

and lungs were detected for wet /dry weight. [ Results[ PCT
increased with the extension of the time within 24 h, the
differences were significant
( H=17.97-21.60,q=4.08-6.88,P<0.01 ) . Compared with the
untreated group, that of treated groups decreased at8 h, 16 h,
and 24 h (H=26.43-27.44,q=4.08-6.27,P<0.01). No difference
was found between HCA group and HS group (P>0.05). Compared
with the untreated group, the PCT in LCA group declined
( t=23.37,28.32;H=15.16,19.55;q=4.08, 4.58;P<0.01 ) ; the
difference of CPR between HCA group and HS group at 24 h was not
significant (P>0.05) W/D of the lungs in vulnerated group
reached the top at 8 h and somewhat lowered at 16 h then recovered
slightly at 24 h, the difference was significant as compared with
the control(H=23.15-24.70,q=4.28-6.87,P<0.01) Conclusion .
50.0 g/L albumin can reduce lung wet /dry weight in early stage
of ARDS, lower CRP and PCT, relieve inflammation and conduce to
early treatment of ARDS.
