PPH 在重度痔疮治疗中的临床应用

【摘要】 目的 探讨 PPH 在治疗重度痔疮中应用的临床价值。

方法 采用美国强生公司或国产的圆形吻合器对 76 例Ⅲ、Ⅳ期脱垂性

痔疮进行手术治疗。结果 手术平均时间 19min，术后住院时间 2～8

天，全组患者术后痔疮回缩，随访 2～36 个月，无复发，无大便失禁、

肛门狭窄或肛周脓肿形成等并发症。结论 PPH 手术具有安全、有效、

术后痛苦少、恢复快、并发症少等优点，是一种治疗重度痔疮较好的

新方法。

【Abstract】 Objective To explore the clinical value of

application of procedure for prolapse and hemorrhoids（PPH）

in the treatment of hemorrhoids at the third or forth

degree.Methods 76patients with Ⅲ ～ Ⅳ degree pro ˉ lapse and

hemorrhoids underwent PPH were treated by a regular stapler made

in China or by John&John Co.Results The mean operating time

was19minutes and hospital stay ranged from2～8days.Hemorrhoids

in all cases disap ˉ peared after operation.No hemorrhoid

recurrence and complications were found during the follow-up

period of2～36months.Conclusion PPH is a safe and effective

treatme nt for hemorrhoids at the third and fourth degree with

the adˉvantages of less postoperative pain，quicker recovery

and lower complieation.

我院自 2000 年 1 月以来，收治Ⅲ、Ⅳ期痔疮患者 76 例，应

用吻合器痔上粘膜环切术（PPH）治疗，经临床观察及随诊分析，结合

有关文献报告如下。

1 资料与方法

1.1 一般资料 全组 76 例患者临床诊断均为Ⅲ、Ⅳ期痔疮患

者，男 45 例，女 31 例。年龄 20～74 岁，平均年龄 41.5 岁。其中Ⅲ

期患者 49 例，Ⅳ期 27 例。主要症状:有肛门不适 76 例，便血 51 例，

有痔块突出的 76 例;病程 1～20 年，平均 8.1 年。既往有 9 例曾用硬

化剂治疗，12 例曾行痔切除术，经药物治疗 73 例。合并有直肠脱垂 2

例，慢性结肠炎 11 例，前列腺增生 5 例。

1.2 器械 采用美国强生公司生产的 HCS 或国产的圆形吻合

器（33mm）及其附件。

1.3 麻醉及体位 选择局部浸润麻醉或骶管阻滞麻醉。 采用膀

胱截石位。

1.4 手术步骤 常规碘伏消毒会阴及直肠，充分扩肛，应用

PPH 器械装置，用 2-0prolene 线，在齿状线上 3cm 直肠粘膜 3 点处行

荷包缝合，再距齿状线上 1～2cm 直肠粘膜 9 点处行另一荷包缝合。缝

合达粘膜下层，进针尽量靠近，避免漏缝粘膜。置入痔吻合器，收紧

荷包线，在持续牵引荷包线的同时，旋紧吻合器，指示刻度到安全域

的底端。击发吻合器后，在关闭状态留置 30s，旋松吻合器并取出，

以半圆规检查吻合口，如有出血用 3-0 可吸收线缝合止血。取出肛管

扩张器后，如肛缘有较大的皮赘或肥大的肛乳头，予一并切除，小的

皮赘无需处理。
