中药饮片自查报告
我单位名称为北京全胜医院，位于北京市朝阳区黄厂南里3号院1号楼。为贯彻北京市食品药品监督管理局今年下发的《通知》要求，我单位对内部中药饮片管理进行了自查，情况如下：
1. 中药饮片质量管理人员状况：
负责中药饮片的质量管理员是中药药剂师、中药师，具有多年从事中药饮片管理工作经验，应对中药饮片质量问题有一定的判断能力，可监督、检查和指导药品购进、验收、养护、保管、销售、运输等各个环节存在或反馈的质量问题。在药品质量检查和养护工作中，药房员工一贯坚持预防为主的原则，根据库存药品流转情况和季节变化，确定重点养护工作。在中药饮片验收过程中，验收员在我单位从事验收工作多年，工作认真、仔细，能及时准确完成所购进药品的质量检查验收工作，并已做过中药饮片验收工作的岗前培训。
2. 中药饮片购进管理：
（1） 将执行“按需进货、择优选择、质量第一”的原则，注重药品购进时的时
效性和合理性，力求做到供应及时，结构合理。
（2） 必须从具有资质的生产、经营企业采购中药饮片，验证生产、经营企业的
《药品经营（生产）许可证》、《企业法人营业执照》、《药品经营（生产）质量管理规范认证证书》、《药品GMP认证书》、药品质量保证协议和销售人员授权委托书，资格证明、身份证，并将复印件存档备查。
（3） 所购的中药饮片要求有包装，包装上除有品名、生产企业、产地、生产日
期外，实施文号管理的中药饮片必须注明批准文号。
（4） 供货企业须提供有中药饮片的检验报告复印件，签订合同都注明质量条款、
产地、等级标准等。
（5） 不购入该炮制而未炮制的中药饮片。
3. 中药饮片验收管理：
（1） 验收人员严格按照法定的药品质量标准和《中国药典》对中药饮片质量进
行逐批验收。
（2） 中药饮片的验收若发现问题，及时向上级汇报，尽快处理。
（3） 验收时仔细核对品名、规格、数量、产地、生产企业、生产批号、生产日
期、合格证等，同时对中药饮片的包装、标签以及有关要求的证明文件均进行逐一检查。
（4） 中药饮片进货时，及时让供货厂家提供饮片检验报告，以便留存备查。
（5） 验收时对与货单不符、质量异常、包装、标志内容不符合规定等存在疑问
的品种，采取拒收，并与进货厂家及时沟通联系，尽快处理。
（6） 验收完毕，验收员在验收货证上签字，验收记录保存存档。
4. 中药饮片的储存、养护管理：
（1） 在库中药饮片应定期采取养护措施，每季度要将全部中药饮片检查一遍，
遇到潮湿季节，每月要将中药饮片检查一遍，出现质量问题，立即采取补救措施。
（2） 中药饮片储存于阴凉处，避光、阴凉、干燥、通风，与其他药品分开存放，
并有防潮、防霉、防尘、防污染以及防虫、防鼠等设施。
（3） 中药饮片容易发生质量变异，养护过程中严格按照其不同性质和储存条件
分别养护。
（4） 性质相互影响容易串味的重要饮片分开存放。
5、中药饮片的处方管理规定：
（1）处方前记包括：医疗机构名称、患者姓名、年龄、性别、科别、费别、诊断以及患者一般信息，门诊或住院号及病区号和床号，麻醉、一类精神药品处方还应记载患者身份证号、家庭常住地址和联系电话等。
（2）处方正文包括：药品通用名，饮片炮制方法，特殊调剂，煎煮方法，剂量等。
（3）处方后记：处方服用天数，日几剂，每剂煎几次，制备方法，服用方法，处方医生签字，调剂人签字，核对人签字，处方金额。
在此次自查行动中，仍然存在一些问题和不足，我单位将严格遵守药监部门要求，积极进行业务学习与自查工作。我院经营的中药饮片不存在改换标签的行为，不存在前店后厂的行为，不存在经营召回产品的行为，我院中药饮片均为经营范围内的合法经营。
以上是我单位依据《药品质量管理规范实施细则》及《通知》等文件精神进行的中药饮片质量管理自查总结，请豆各庄食药所检查指导！

关于中药饮片管理自查情况汇报
为加强中药饮片管理，确保科学、合理、安全、准确地经营中药饮片，杜绝销售假药、劣药，根据《药品管理法》及《药品经营质量管理规范》及其细则要求，现将公司贯彻实施中药饮片检查具体情况汇报如下：
1、 加强中药饮片购进管理
药品进货是药品经营的第一关，是药品流通中质量管理至关重要的一个环节，而首营企来和首营品种是该环节管理的重点。医院规定由采购部在电脑中输入首营企业和首营品种的基本信息，输出首营企业、首营品种审核表，质管部对供货方的资质进行审核，经有关人员和领导审核批准后才能进货；真正把住药品购进的审核表，确保医院购进药品为合法企业所生产或经营的合格药品。
①所购中药饮片必须是合法生产企业生产的合法药品；

②所购中药饮片应有包装，包装上应有品名、规格、生产企业、生产日期，实施批准文号管理的中药饮片还应有药品批准文号和生产批号；
③购进进口中药饮片应有加盖供货单位质量管理机构原印章的《进口药材批件》及《进口药材检验报告书》复印件；
④该炮制而未炮制的中药饮片不得购入。
⑤不得外购散装饮片，加工包装等行为。
2、 强化中药饮片入库验收管理
在药品验收环节上，为了切实做到验收按程序规范操作，把好药品入库第一关。医院首先从资源配备入手，由质管部对验收人员进行培训，给验收员讲解制度和程序文件，提高验收员的质量意识。同时加大监督管理力度，质管部人员到验收第一线，抓质量管理制度、程序的实施和落实，养成规范操作的习惯。重点对验收员进行岗位检查和考核。 ①验收员应按照法定标准和合同规定的质量条款对购进的中药饮片进行逐批验收； ②验收时应同时对中药饮片的包装、标签及有关要求的证明或文件进行逐一检查； ③验收应按照规定的方法进行抽样检查；
④验收应按规定做好验收记录，记载供货单位、数量、到货日期、品名、规格、生产厂商、生产日期、质量状况、验收结论和验收人员等项内容；实施批准文号管理的中药饮片还应记载药品的批准文号和生产批号；
⑤分斗包装合格证要放在斗里，没有合格证就按照假药处理；
⑥验收记录应保存至超过药品有效期一年，但不得少于二年；
⑦对特殊管理的中药饮片，实行双人验收制度。
3、加强中药饮片储存管理
在库药品的存放条件和科学的养护方法，对药品安全的储存，保证质量稳定起重要作用。在日常工作中，我们对在库药品进行科学养护，合理储存。根据药品储存条件和药品的属性，将药品分别存放于相应的库区中；按药品的属性分类存放；养护工作采取质量普查和重点养护相结合的方法，对仓库的药品进行科学养护，在养护中切实有效的行使否决权；对近效期药品按月报《近效期药品催销表》坚持每天两次定时检查，记录仓库温湿度，根据具体情况采取空调降温、除湿或洒水增湿等措施，保证药品储存条件符合其储存的要求，药品质量得到根本保障。
①应按照中药饮片储存条件的要求专库、分类储存，按温湿度要求储存于相应库中，易串味药品应单独存放；
②中药饮片应按其特性采取干燥、降氧、熏蒸等方法养护，根据实际需要采取防尘、防潮、防污染以及防虫、防鼠、防鸟等措施；
③中药饮片定期采取养护措施，按季度对饮片全部巡检一遍。夏防季节，即每年5—9月份，每月要将全部饮片检查一遍；
④中药饮片装斗前应进行装斗复核，不得错斗、串斗，并做好记录；
⑤中药饮片装斗前应进行净选、过筛，定期清理格斗，饮片前应写正名、正字，防止混药；
⑥饮片上柜应执行先产先出、先进先出，易变先出的装斗原则；
⑦每天应校对所有衡，工作完毕整理营业场所，保持柜内外清洁，无杂物；
⑧不合格中药饮片的处理按有关制度执行处理，严禁不合格药品上柜销售；发现质量问题，应立即报告质量管理中，并采取有效措施。
以上几方面是中药房实施中药饮片工作的自查汇报。

二．培训的及要求培训目的
安全生产目标责任书
为了进一步落实安全生产责任制，做到“责、权、利”相结合，根据我公司2015年度安全生产目标的内容，现与财务部签订如下安全生产目标：
一、目标值：
1、全年人身死亡事故为零，重伤事故为零，轻伤人数为零。
2、现金安全保管，不发生盗窃事故。
3、每月足额提取安全生产费用，保障安全生产投入资金的到位。
4、安全培训合格率为100%。
二、本单位安全工作上必须做到以下内容：
1、对本单位的安全生产负直接领导责任，必须模范遵守公司的各项安全管理制度，不发布与公司安全管理制度相抵触的指令，严格履行本人的安全职责，确保安全责任制在本单位全面落实，并全力支持安全工作。
2、保证公司各项安全管理制度和管理办法在本单位内全面实施，并自觉接受公司安全部门的监督和管理。
3、在确保安全的前提下组织生产，始终把安全工作放在首位，当“安全与交货期、质量”发生矛盾时，坚持安全第一的原则。
4、参加生产碰头会时，首先汇报本单位的安全生产情况和安全问题落实情况；在安排本单位生产任务时，必须安排安全工作内容，并写入记录。
5、在公司及政府的安全检查中杜绝各类违章现象。
6、组织本部门积极参加安全检查，做到有检查、有整改，记录全。
7、以身作则，不违章指挥、不违章操作。对发现的各类违章现象负有查禁的责任，同时要予以查处。
8、虚心接受员工提出的问题，杜绝不接受或盲目指挥；
