供货商承诺书
我方（供方）经与贵方（需方）平等协商，并按贵方要求，贵我双方就合＿＿ ＿＿ ＿＿ （ ）供货商： 作事宜已达成一致：本单位（本人）保证以合法身份与贵方（武汉华工后勤管理有限公司下属）合作，通过合法方式向贵方供应安全合适、质优价廉的物资。
在双方协议基础上，为了明确供需关系并达到贵方要求，我方特向贵方作出以下供货承诺：
一、向贵方提供本单位由政府核发的有关合法证件，即营业执照、生产许可证、卫生许可证或个人健康证、产品检验合格证书或身份证以及其他必须的专业证照等（原件审查、复印件备案）。
二、本单位（本人）自愿以月结货款作为对贵方的质量、信誉保证金。
三、质量标准：为贵方提供与所供产（商）品的国家质量标准或相应等级质量价格报告单相符的物资。
四、供货时间：以贵方确定的时间为准，保证按贵方订购物资的数量、质量要求按时到位。
五、验收方式：数量验收以贵方（食堂、餐厅）实际验收净重量或单位数量为准；质量验收：所供产（商）品质量、品种、品牌、规格及产地经贵方验收认可为准，若质量不合要求，承诺包退包换、并及时补充配送到位。
六、严格按贵方规定的定价原则执行，并在随行就市前提下及时诚信报价。
七、出现下列情况，除按贵方的规定扣除相应质量保证金外，按相关规定自愿放弃供应商资格。
1、不按贵方提供的订货单（或电话订单）中的品种、数量、质量要求进行配送，造成贵方损失和不良影响的，每次罚扣100元，一月内累计出现三次类似情况者，自愿放弃供应商资格。
2、如配送物资出现质量问题，经贵方采供部确认每次自愿扣罚300元质保金，一月内累计达两次者自愿放弃供货资格。
3、配送物资严格按贵方规定的品牌、等级、种类物资供应，不得自行改变供货类别、降低供货等级、变更品种名称和虚开数量套开其它品种。如违反本规定除不报账外，愿按违规金额的5倍罚款，情节严重（数额较大）的自愿放弃供货资格。
4、配送物资因质量问题退货不到位，一月内累计达两次自愿放弃供货资格。
5、配送物资时服从贵方管理，遵守贵方有关规定，做到配送服务周到，不无理取闹，打架斗殴，不擅自进入食堂、餐厅操作间私拿物品，不欺行霸市、强买强卖，如有以上情况，自愿取消供货资格。
6、配送的物资无论什么原因，造成食物中毒的，无条件由贵方扣除全部质量保证金，并自愿承担相关法律责任和赔偿经济损失，永远放弃供货资格。
7、自觉接受贵方食堂、餐厅及使用单位每月对本供应商配送物资的综合评议，对反映的问题高度重视、及时解决，不断提高配送质量，如供货期间累计综合评议不合格达二次，愿自动取消供货资格。
8、因我方作为供货商造成违约、失信或
违规配合贵方内部人员的，自愿接受贵方相关处罚，绝不寻找任何借口干扰贵方的正常供应工作和服务保障秩序。
9、如果贵方因自身调整需要而决定改变采购模式和供货渠道，本单位（本人）无条件服从贵公司单方面提出解除供货关系的决定并予以主动配合。

供货保障措施及组织能力介绍
为保证为贵项目工程及时供货，特拟定以下保障措施：
一、组建产品供应项目部
针对本工程，单位抽调经验丰富、责任心强的业务骨干组建产品供应项目部。负责本工程的产品供应。

三、供货流程：
1、货物采购
按照合同约定，项目经理组织采购。
2、到货检验
到货后，甲方和监理及丙方参加该产品在供货商所在地或交货地进行的产品到货检查。检查前我方将提前通知甲方和监理及丙方派员参加。
3. 开箱检验
在仓库或工地现场进行的开箱检验由甲方、监理、丙方我公司等有关人员共同参加，对产品的内外包装产品外观进行检查，若发现产品短缺和外观破损，我公司将及时处理。
4. 安装验收
安装验收是我公司和施工单位共同对产品安装工程根据有关的产品安装技术指标进行验收。安装验收后双方签署安装验收证书。
5. 完工测试
系统完工测试主要是对已安装产品进行单体测试。
6. 大联调
本系统产品联调是指：在与其它系统不相连的情况下，测试所有产品作为一个完整的系统能否很好地工作，能否完成系统每一个功能要求，是否符合贵方的要求。
系统功能测试完毕后，签署调试验收证书。
7. 试运行
系统运行测试完成，到系统现场完工测试结束，试运行验收通过后，系统将移交甲单位有关部门。
8. 质保期
系统验收合格后开始进入质保期，正式投入使用，期间将进行抽查各种测试项目，检查系统的可靠性和稳定性。
产品质保期按质保及售后服务承诺书介绍进行质保。
四、供货质量标准：
我方将严格按以下标准提供产品
1．产品质量标准按产品技术要求并符合国家有关规定。
2．产品的规格尺寸按甲方提供的技术资料要求执行。
3. 送货时需提供材料的合格证、检测报告（报告必须出自国家权威检测机构）原件一式叁份。每批货物进场，我方必须出具产品合格证，甲方、丙方、监理方等签收后留存。
五、供货数量发生变化处理方法
1、如因工程需要，供货量变化超出暂定供货量的±10%时，甲方以书面通知我方，此项通知甲方应充分考虑我方的合理备料和加工周期，具体增减数量以甲方书面通知为准，。
2、因甲方要求的产品加工方式及原材料品种变更而引起变更产品（即合同中未约定的产品）供货期由我方、甲、丙方另行协商确定，且供货不得影响工程的相应进度。
六、技术支持
