须 知

1.本申请书一个认证单元应填写一份申请书并一式二份寄/送至公安部消防产品合格评定中心, 第三份自己存档。

2．首次申请须提交《消防产品认证工厂条件调查表》(CCCF/002-2002(A/1))；
3．《机动车辆类（汽车消防车产品）强制性认证实施规则》(CNCA-02C-023：2002/A1)在发布；本申请书、合同、消防产品认证工厂条件调查表等文件通过http：//下载；

4．其他提交材料一式二份寄/送至公安部消防产品合格评定中心：
1） 申请人及生产厂的工商注册证明复印件（加盖印章）；
2） 申请人及生产厂的组织机构代码证复印件；
3） 提交所申请商标的注册证明及商标使用授权书；
4） 本申请表应用中文填写。
5. 非首次申请如实填写已申请并通过认证的产品情况。

1. Applicant should fill in an application in a unit and send two filled copies of this Application Form to China Certification Center for Fire Products (CCCF), and the third copy should be filed by the applicant.

我们声明我们将遵守《机动车辆类（汽车消防车产品）强制性认证实施规则》（编号：CNCA－02C－023 ：2002/A1）和公安部消防产品合格评定中心的公开文件规定，支付认证所需的申请、检验、工厂审查及其它有关的费用，理解并接受公安部消防产品合格评定中心提出的评价活动计划；公安部消防产品合格评定中心将不承担获得认证产品的生产厂或销售商应承担的任何法律责任。
We are hereby to declare that we will follow the 3C certification rules（CNCA－02C－023：2002/A1）and procedures of CCCF and make payment for the fees arising from the application, testing, site audit and other services; understand and accept the evaluation activity plan put forward by CCCF；CCCF will not bear any legal responsibility which should be borne by the factory or salesman granted with the conformity certificate.

申 请 企 业 承 诺

1） 始终遵守《机动车辆类（汽车消防车产品）强制性认证实施规则》（编号：CNCA－02C－023 ：2002/A1）及认证计划安排的有关规定；
2） 为进行评价作出必要的安排，包括审查文件、进入所有的区域、查阅所有的记录（包括内部审核报告）、评价所需人员（例如检验、检查、评定、监督、证书延续）和解决投诉的有关规定；
3） 仅在获准认证的范围内作出有关认证的声明；
4） 在使用产品认证结果时，不得损害公安部消防产品合格评定中心的声誉，不得做使公安部消防产品合格评定中心认为可能误导或未经授权的声明；
5） 当证书被暂停或撤销时，应立即停止涉及认证内容的广告，并按公安部消防产品合格评定中心要求交回所有认证文件；
6） 认证仅用于表明获准认证的产品符合特定标准；
7） 确保不采用误导的方式使用或部分使用认证证书和报告；
8） 在传播媒体中对产品认证内容的引用，应符合公安部消防产品合格评定中心的要求。

Commitment of the Applicant Firm

1. Always comply with the stipulations related to the arrangement in the certification plan;
2. Make a necessary arrangement for evaluation，including documents review，all area entries，checking all records (including internal audit report )，evaluating the needed personnel (e.g. relating to testing，inspection，assessment，surveillance and reassessment)and stipulations related to resolving complaints;
3. Make a Statement related to the certification only in the scope of approved certification;
4. Never ruin the CCCF’s reputation and make any statement which CCCF considers it possibly to cause misleading or be unauthorized when the results of the product certification are applied;
5. Must immediately stop advertisement involving the contents of the product certification and hand back all documents related to the certification in accordance with the CCCF’s requirements when the certificate is suspended or withdrawn；
6. The certification is only used to demonstrate that the product awarded with certificate meets the specific standard;
7. Assure not to adopt a way of misleading to the usage or partial usage of the approved certificate and the test report;
8. It should meet the CCCF’s requirements when the contents of the product certification are to be quoted by mass media.
