Project Location:　 Shanghai Comprehensive Industrial Zone（Fengxian）
监理工程项目范围：一个主厂房（一层，局部二层），一个门卫一个天燃气站及设备安装
Scope of Supervision: One main workshop (one storey, partially two storey) a gate house and a LNG station， installation of utilities.
工程工期： 2006年12月31日
Completion date: December 31st, 2006
工程造价：32,000,000元人民币(General Contractor’s bid )
Construction cost: 32,000,000 RMB
监理服务期：监理合同(及协议)生效之日起至工程通过正式验收、且工程文件完成、政府主管部门的登记备案止。
Supervision Duration：Start from effectiveness of Supervision Contract (and the agreement) to the project is accepted by government; the construction documents are completed and registered by the related government department.

4. 文明施工目标Construction civilization target：创施工文明工地。Make a civilized site
五. 监理工作依据 Basis of Supervision Work
1. 工程合同文件：《建设工程委托监理合同》和 《建设工程施工合同》。
Project contract documents: A Commission Construction Supervision Contract for Project Construction and Contract for Project Construction
2. 设计文件：经过批准的设计图纸、技术说明和其它设计文件，施工组织设计（方案）及设计交底和图纸会审的会议纪要等。
Design document: approved design drawings technical specification and other design document, record of explanation meeting of construction method (or management plan) and design, and minutes of drawing explanation meeting etc.
3. 国家及上海市强制性规范，质量验收规范和有关标准。
The compulsory codes and quality acceptance codes of China and Shanghai and relevant standard.
六. 监理组织机构及监理工程师职责 Organization and the responsibilities of the supervisors
1. 监理的组织结构 Organization
本项目由总监现场代表负责与业主及项目管理公司的日常联系，并领导现场项目监理机构，监督工程项目的正常实施，确保业主和项目管理公司能在任何时间了解项目的实施情况。如业主和项目管理公司发现监理单位的现场管理人员不称职，有权要求监理单位撤换，替换人员需经业主和项目管理公司审定。
TJPM appoints a representative of general supervision engineer to contact with the Bomag and SIP, and organize a site Supervision Team under his leadership; sipervise construction of the project, guarantee Bomag and SIP can know execution of the project at any time. Bomag and SIP can ask the supervision co. to change the personnel who is found unemployable by Bomag and SIP at any time. The personnel to be changed will be examined by Bomag and SIP.
监理是以全职身份负责工程质量控制、协助业主和项目管理公司对工程成本的管理及进度的控制，施工的督导，以确保工程质量合格。
Jianli is in charge of the quality control, cost management, progress control and supervision of construction in full time to guarantee the project quality is ok.
另外，同济派驻工地的监理工程师将全程督导现场的施工。施工进度、问题和解决方法一个月汇报一次，并以标准格式向业主和项目管理公司提出书面报告。
In addition, the Supervision Engineers sent by TJPM will supervise the whole course of construction. The progress, problem and resolution will be reported monthly to Bomag and SIP in standard written report.

2. 监理工程师职责与权限 Liability and purview of Supervision Engineer
总监理工程师职责与权限 Liability and purview of C.S.E.
l 确定项目监理机构人员的分工和岗位职责；
Decide the work and post responsibilities of the supervisors
l 主持编写项目监理规划、审批项目监理实施细则，并负责管理项目监理机构的日常工作；
Organize to prepare the supervision plan, approve the detailed supervision rules, and control the daily work of the Supervision Team.
l 审查分包商的资质，并提出审查意见；
Examine the qualification of the sub-contractor and make comments;
l 检查和监督监理人员的工作，根据工程项目的进展情况可进行人员调配，对不称职的人员应调换其工作；
Check and supervise the supervisors’ work, and dispatch the personnel according to the project progress, change the personnel who are unemployable.
l 主持监理工作会议，签发项目监理机构的文件和指令；
Organize supervision meeting, sign the documents and orders of the Supervision Team;
l 审定总承包商提交的开工报告、施工组织设计、技术方案、进度计划；
Examine the commencement report, construction management plan, technical method and progress schedule submitted by the General Contractor;
l 审查和处理工程变更；
Check and deal with the construction variation;
l 主持或参与工程质量事故的调查；
Organize and take part in investigation of construction quality accident;
l 组织编写并签发监理月报、监理工作阶段报告、专题报告和项目监理工作总结；
Prepare and sign the monthly supervision report, phase report of supervision work, special report and supervision work summary.
l 审核签认单位工程和分部工程的质量检验评定资料，审查总承包商的竣工申请，组织监理人员对待验收的工程项目进行质量检查，参与工程项目的竣工验收；
Check and sign the quality inspection documents of divisional and subdivisional work, check the General Contractor's application of completion, organize to inspect the works to be accepted, take part in completion acceptance;
l 主持整理工程项目的监理资料。

总监理工程师代表职责与权限 The Liability and purview of the representative of Chief Supervision Engineer (C.S.E.)
l 负责总监理工程师指定或交办的监理工作；
Be in charge of the work assigned and handed over by C.S.E.;
l 按总监理工程师的授权，行使总监理工程师的部分职责和权力；
Perform part of the Liability and authority of C.S.E. according to the authorization of the C.S.E.;
l 总监理工程师不得将下列工作委托总监理工程师代表：
C.S.E. will not entrust the representative to do following work:
-----主持编写项目监理规划、审批项目监理实施细则；
Preparation of project supervision plan, approval of detailed supervision rules;
-----签发工程开工/复工报审表、工程暂停令、工程款支付证书、工程竣工报验单；
Signing of commencement order/going back to work order, work suspension order, payment certificate, and completion application;
-----根据工程项目的进展情况进行监理人员的调配，向总监建议调换不称职的监理人员。
Dispatch the personnel according to the project progress; suggest the C.S.E. to change the personnel who is unemployable.
各专业监理工程师职责与权限
Liabilities and purview of all Supervision Engineers of all disciplines
l 负责编制本专业的监理实施细则；
Be in charge of preparation of the detailed supervision rules of his discipline;
l 负责本专业监理工作具体实施；
Be in charge of supervision work of his discipline;
l 组织、指导、检查和监督本专业监理的工作，当人员需要调整时，向总监理工程师提出报告；
Organize, guide, check and supervise the construction supervision of his discipline, apply to C.S.E. when the personnel is needed to be changed;
l 审查总承包商提交的涉及本专业的计划、方案、申请、变更，并向总监理工程师提出报告；
Examine the plan scheme, application, variation which involving with the discipline, and report to the C.S.E.;
l 负责本专业分项工程验收及隐蔽工程验收；
Be in charge of acceptance of subdivisional work and concealed works;
l 定期向总监理工程师提交本专业监理工作实施情况报告,对重大问题及时向总监理工程师汇报和请示；
Report the C.S.E. regularly the supervision work of his discipline, and report the C.S.E. the important problems ask for instruction;
l 根据本专业监理工作实施情况做好监理日记；
Write diary of the supervision work of his discipline;
l 负责本专业监理资料的收集、汇总及整理，参与编写监理月报；
Be in charge of collecting, summarize and sort out of the supervision documents of his discipline and take part in preparation of monthly supervision report;
l 核查进场材料、设备、构配件的原始凭证、检测报告等质量证明文件; 及其质量情况，根据实际情况认为有必要时对进场材料、设备、构配件进行平行检验，合格时予以签认；
Check the quality certificates such as original certificates of material, equipment, component brought onto site and the inspection certificates, parallel inspection can be made for the material, equipment and component if necessary, and sign when the quality is OK;
l 负责本专业的工程计量工作，审核工程的数据和原始凭证。
Be in charge of measurement of the his discipline and check the construction data and original certificate;
监理员职责与权限 Liability and purview of the Supervision Engineer’s assistant
l 在专业监理工程师的指导下开展现场监理工作；
Carry out supervision under direction of the discipline engineer;
l 检查总承包商投入工程项目的人力、材料、主要设备及其使用、运行状况，并做好检查记录；
Check the manpower, material, main equipment of General Contractor and use, operation of them, and make record;
l 复核或从施工现场直接获取工程计量的有关数据并签署原始凭证；
Recheck or obtain directly the measurement data on site and sign the original certificate;
l 按设计图有关标准，对承包商的工艺过程或施工工序进行检查和记录，对加工制作及工序施工质量检查结果进行记录；
Check and record the Contractor’s construction or the process in accordance with the drawing, check and record the fabrication and quality;
l 担任旁站工作，发现问题及时指出并向专业监理工程师报告；
Undertake the stander-by supervision, and inform the General Contractor and report the engineer timely when problem is found;
l 做好监理日记和有关的监理记录。
Write supervision diary and concerning supervision record.

文档管理员 Archivist
文档管理员是在总监领导下开展工作，其工作对总监负责，其主要任务是：
Archivist works under leadership of C.S.E., and answers for C.S.E.:
l 管理各类收发文资料，并归类、整理；
Control all the received and delivered documents and collect and sort out them;
l 完成各类文档处理工作；
Complete treatment of all sorts of documents
l 完成监理组内部的日常行政事宜。
Complete internal daily administration matter of the Supervision Team.
3. 监理人员的素质要求 Requirement of supervisor’s quality
工程监理是一项责任性很强的工作，因此要求监理人员在职业道德、技术方面、工程管理方面具备良好的素质。因此，本工程监理人员的选择主要考察了这三方面：
Construction supervision is a work with high responsibility; it requests the supervision personnel should have good quality in morality, technology, project management. So we chose the supervision personnel from these three aspects:
l 工程技术方面 Technology aspect
在工程技术方面，本项目组的监理人员具有本专业的理论基础和比较丰富的设计和施工经验。
In construction technology, the supervision personnel of this team have the theory foundation and rich experience of design and construction of his discipline.
l 工程管理方面 Project management aspect
本项目的监理人员均参加过监理工作和监理培训，具备工程管理方面的知识和才能，包括现代化的管理手段和合同管理意识。
The supervision personnel for this project has participated supervision work and training of supervision, has knowledge of project management and ability including modern management means and contractual management sense.

l 职业道德方面 Morality aspect
监理人员必须秉公办事，按照合同条件公正地处理各种问题，遵守国家的各项法规。既不接受业主和项目管理公司所支付的酬金之外的任何回扣、津贴和其他间接报酬，也不得与承包商有任何经济往来，包括接受承包商的礼物，不得经营或参与施工以及设备、材料的采购活动。
Supervision personnel will deal with all kinds of problems fairly and obey all the rules of laws. Neither accept any return commission beyond the supervision service fee, subside and other reward nor have any economic relationship with the Contractor including gift, nor have business of or take part in construction and procurement of equipment and material.
七. 监理工作程序及流程 Supervision process and work flow
1. 监理工作程序 Supervision process
l 根据监理工作范围、监理工作内容编制工程项目监理规划；
Prepare supervision plan according to the supervision scope and content;
l 按工程建设进度、分专业编制工程建设监理细则；
Prepare detailed supervision rules according to the construction progress and the discipline;
l 按照建设监理规划和建设监理细则展开工程建设监理活动；
Carry out supervision in accordance with the construction supervision plan and detailed supervision rules;
l 按照规定的作业程序进行监理；
Supervise in accordance with the specified operational procedure;
l 签署分部工程单位工程质量证明文件；
Sign quality certificate document of divisional work and subdivisional work;
l 遵守总承包商自评、设计勘察认可、监理核定、业主验收的顺序，参与工程预（初）验收并出具书面的《工程质量评估报告》；
Follow the procedure that General Contractor evaluates first, design institute confirms, supervisor checks and confirms, IKS accepts, take part in pre-acceptance (initial acceptance) and provide quality evaluation report.
l 完成监理业务后，向业主和项目管理公司提交项目监理资料。
Provide Bomag and SIP supervision documents after supervision is completed.
