一、监理依据

一、监理依据
　　在监理处监理范围内的涵洞施工监理应以下列文件为依据：
　　1、业主与承包人之间的《工程承包合同》及《补遗书》。
2、业主与监理单位之间的《监理委托合同》及《补遗书》。
　　3、JTGF80／1-2004《公路工程质量检验评定标准》和有关试验规程。
　　4、本工程的《招标文件》有关章节。
　　5、本工程设计文件及有关设计技术联系单。
　　6、JTG/T F50－2011《公路桥涵施工技术规范》。
　　7JTG E30－2005《公路工程水泥混凝土试验规程》。
　　8、业主和总监办以及上级质检部门的各项通知等规定。
　　9、被批准的《施工组织设计》和《分项工程开工报告》。
二、工程概况
1、工程简介
本项目位于郴州市北湖区和桂阳县境内，路线呈东西走向，起点位于北湖区同心桥，起点桩号K0+000，路线沿X090老路布线，向西经高壁（与新107交叉），寒溪铺(下穿武广高铁)、秀水、塘昌铺，在华塘铺处绕避镇区走新线,在招旅村处跨越溪河,终点位于郴州市北湖区与桂阳县交界处,终点桩号K19+500,路线全长19.474283KM。
主要控制点为北湖区同心桥、高壁、寒溪铺、塘昌铺、华塘铺、招旅村。

三、施工准备阶段的监理
　　1、监理工程师按图纸或变更通知所规定的涵洞位置、方向与主线的交角，标高等项目，检查承包人的放线结果。　　
　　2、施工前，监理工程师审查承包人提交的分项或分部工程开工报告。
3、现场清理完毕，并经监理工程师检查同意后方可开工。　
四、基坑开工
1、基底平面尺寸应符合设计与技术规范要求。
2、基底标高应符合设计与技术规范要求，对于土质基底宜预留20-30mm厚的土层，在施工垫层前突击挖除，防止被水浸软，基底不得超挖。
3、坑底应设排水沟、集水坑等，以便及时排水。
4、基底坑壁坡度应根据地质情况选择，保证基坑的安全，必要时需要取支挡措施。
5、基坑开挖长度应根据施工组织施工能力确定，不能盲目大面积开挖，尽量减少基坑的暴露时间。
五、地基承载力检测
1、开挖好的基坑必须进行地基承载力试验，监理进行旁站或做平行试验。
2、地基承载力应大于或等于设计要求值。若地基承载力不能满足设计要求，承包人应提出提出处理方案报监理工程师，由监理工程师组织按变更程序确定处理方案，承包人按批准的方案进行地理处理。
六、盖板涵
1、模板制作安装
应要求施工单位宜采用标准化、系列化、通用化的组合模板、制作的模板应保持其表面平整，形状准确、不漏浆、有足够的强度和刚度。
在检查模板的安装质量时，应注意模板的平面位置和尺寸，符合设计图纸和规范要求的范围。在模板使用拼装前，应要求施工单位将模板面清理干净，并均匀涂刷脱模剂两遍。外露砼模板的脱模剂应采用同一种，不得使用废机油等涂料，且不得污染钢筋及砼的施工缝处，在砼施工时，应安排专人负责随时观测、检查，针对各种突发情况及时采取相应的应急措施。
模板安装完毕后，对其平面位置、几何尺寸、顶部标高、节点联系及纵、横向稳定性进行检查，会签后方可浇筑砼。
2、钢筋制作、安装
①钢筋原材料控制
⑴钢筋进场时应对应加强外观检查，要求钢筋的表面洁净、无损伤，在使用前清除油渍和铁锈等。
⑵参照《公路工程金属试验规程》（JTJ005）有关的规定进行钢筋的可焊性试验，确定钢筋的焊接性能。
⑶针对钢筋的不同品种、等级、牌号、规格及生产厂家，必须做到分批验收，分别存放，且应设立标示牌。钢筋的宜堆置在仓库（棚）内，露天堆放时应垫高并遮盖。
⑷对于有严重曲折的钢筋，使用前将弯折处校直，对于局部校正不直，或由于反复弯曲产生裂纹的钢筋不得使用。
⑸钢筋应具有出厂质量证明书和试验报告单，并应抽取试样做力学性能试样。
②钢筋加工控制
⑴监理工程师对钢筋加工的控制，除应按相应规范要求进行检查验收外，还要在加工钢筋前对图纸的工程数量进行详细的复核，包括钢筋型号、根数及长度等。同时必须对混凝土保护层，钢筋弯曲类型、弯钩等确定相应的下料长度。
⑵盘园钢筋应采用冷拉方法调直钢筋，并控制冷拉率或控制应力。
⑶钢筋焊接必须是具有有效焊工考试合格证，并在合格证允许的操作范围内生产。
⑷必须按设计要求选择焊条、焊剂，各种焊接材料性能应符合现行《钢筋焊接及验收规程》（JGJ18-2003）规定。
⑸钢筋焊接在雨雪，大风天气和气温低于-20°C时不得进行焊接作业。
⑹钢筋焊接及绑扎接头应设置在受力较小处，并错开布置。接头长度及焊头断面面积，接头断面之间距离应符合规范规定。
③钢筋骨架或网片安装控制
⑴钢筋的安装应尽可能先制作成网片，放入模板内焊接或绑扎成整体。
⑵对有焊接接头或绑扎接头的钢筋应检查每根接头质量及位置。
⑶应在钢筋与模板间设置垫块，垫块应与钢筋扎紧，并相互错开，钢筋保护层厚度，应符合设计和施工规范规定。
⑷在砼浇筑前，应对已安装好的钢筋的品种、规格、数量、安装质量以及预埋件进行检查。
3、混凝土拌合、浇筑
①水泥质量控制
⑴袋装水泥在运输、贮存过程中，应防潮，不同强度等级、品种和出厂日期的水泥应分别堆放。
⑵水泥受潮或时间超过3个月，应重新取样检验，并按复检结果执行。
⑶水泥应符合现行国家标准，并附有生产厂的水泥品质试验报告，水泥应按规范要求进行复查试验。
⑷水泥存放应采取防雨、防潮措施。
②粗、细集料应符合施工技术规范规定，并按现行《公路工程集料试验规程》（JTG E42-2005）进行试验。
③拌合用水可用任河河水或山泉水。
④混凝土拌制要求
⑴严格按监理工程师批复的配合比进行配料，并根据原材料含水量变化，及时调整施工配合比，各种原材料计量应准确。要求水泥、水、外加剂的计量允许偏差为±1%；粗、细集料计量允许偏差±2%.
⑵混凝土应使用强制式拌合机集中拌合、拌合时间不少于90s。
⑶混凝土拌合物应拌合均匀，不得有离析和沁水现象。
⑷应在搅拌站和浇筑地点分别取样检测砼坍落度。坍落度检查频率应符合规范规定。
⑸宜用搅拌运输车运输混凝土(途中应以2-4r/min慢速转动)。
⑹混凝土运至浇筑地点后发生离析、泌水或坍落度不符合要求时，不得任意加水，确有必要时，可同时加水和水泥以保持原有水灰比不变。如二次搅拌仍不符要求，则不得使用。
⑺对于超过时间、工作性能已不能满足施工要求的砼必须废弃，不能用加水的方法改变砼的坍落度。
⑤混凝土的浇筑
⑴自高处向模板内倾卸砼时，为防止离析，其自由倾落高度不能超过2m，当倾落告诉超过2m时，应使用串筒。
⑵混凝土应按一定厚度，顺序和方向分层浇筑，应在下层混凝土初凝或能重塑前浇筑完成上层混凝土。
⑶混凝土振捣用插入式振捣器，振捣必须均匀、充分，但不得过振。应避免振捣棒触及模板、钢筋及其他预埋件。
⑷混凝土浇筑应连续进行，如因故间断时间超过混凝土初凝时间，应预留施工缝。施工缝留设位置应符合规范或设计要求。
⑸混凝土浇筑过程中或浇筑完成时，如混凝土泌水较多需采取措施及时排除泌水，并查明原因，要采取措施，减少沁水。
⑹结构混凝土浇筑完成后，对混凝土表面及时进行修整、抹平，待定浆后再抹第二遍并压光或拉毛，并及时覆盖。
⑺混凝土终凝后应及时洒水养生，养生用水要求同混凝土拌合用水。养生时间一般为7天，当掺加外加剂时需养生14天，其它气候和条件下的养生应符合相关规定。
4、沉降缝位置应符合设计和施工规范要求，竖直、上下贯通、没有错位或弯曲、填缝无空鼓，开裂及漏水现象。
5、帽石、一字墙式八字墙应平顺，与路线边坡。线形匹配。
6、台身盖板支撑处必须平整，避免高低不平或混凝土表面毛糙，导致盖板在支撑处受力不匀。
7、采用预制盖板施工时，应注意检查上、下面的方向和斜交角度的方向，避免发生反向错误。

涵洞施工控制要点
1、监理工程师按图纸或变更通知确定的涵洞位置、方向，与主线的交角、标高等项目，检查承包人的放线结果。

5、钢筋的制作与安装应严格按图纸要求和规范规定进行施工，钢筋的保护层应符合要求。
6、模板安装应牢固，接缝紧密、模板涂刷脱模剂应均匀、颜色一致，模板安装后应对模板标高、几何尺寸、平面位置进行认真复核，并报监理工程师验收。
7、涵洞基础、墙身砼应按设计沉降缝位置分段浇筑。砼拌合必须采用强制式拌合机集中拌合，振捣应密实、均匀。
8涵身沉降缝位置应与基础上、下一致，竖直、不错台、不弯曲、沉降缝处理应符合设计及规范要求。
9、涵台帽在盖板支撑处必须抹平，保证盖板在支撑处受力均匀。涵台帽砼强度达到设计强度75%后方可进行盖板安装。
10、台背宜填天然级配砂砾石。当涵洞盖板安装完成后，并经检查验收后，既可进行台背回填。回填应水平分层，两侧对称按设计或规范要求进行填筑。
11、涵洞顶填土厚度必须大于0.5-1m时，才能允许机械通过。
