(1) 建造合同日期定义Definition of date of “Contract for Construction “ as per IACS PR 29 （Rev.2）：
The date of “Contract for Construction “of a vessel is the date on which the contract to build the vessel is signed between prospective owner and the shipbuilder. This date and construction numbers (i e Hull Numbers) of all vessels include the contract are to be declared to the classification by the party applying assignment of class for a new building.
建造合同日期是由预期的船东和船厂之间签署一艘船舶的建造合同日期。其合同（涉及所有船舶）日期和船舶建造工程号（如船厂编号）应由申请建造入级的申请方向入级船级社提出书面声明。
（2）系列姐妹船（a series of sister vessels）：
“a series of sister vessels” is a series of vessels built to the same approved plans for classification purpose, under a single contract for construction.
系列姐妹船是在一个单一合同中标明的按照相同批准图纸建造的船舶。
（3）选择权船舶（Optional ships）：
Optional ships will be considered part of same series of sister vessels if the option is exercised not later than 1 year after the contract to build the series was signed.
选择权船舶将被认为属于系列姐妹船的一部分，条件是其选择权在系列船舶合同签署之后1年之内生效。
（4）填写说明（Instruction for filling）：
a. 如果签署一个建造合同包括实际船舶艘数，没有选择权船舶，填写A项。
A single contract for construction is signed without any optional vessels, write in item A.
b. 如果签署一个建造合同包括实际船舶艘数和选择权船舶，如5+5，则填写B项（前5艘船舶）。
A single contract for construction is signed with optional vessels, i.e. 5+5 vessels, first 5 vessels to be writed in item B.
c. 上述合同中说明的选择权船舶且在1年中生效，如+5船舶则填写B+C项。
Optional vessels exercised within 1 year after the single contract for construction a series of sister vessels i.e. +5 vessels, write in item B+C.
d. 申请方填写建造入级申请并签署、盖章，认为属于（1）的书面声明。
Application for New Building Ship’s Service with signature and seal by applicant is regard as declaration from applicant as mentioned in (1)
e. 对图纸审批，本申请适用于系列姐妹船；对建造检验，本申请适用于1艘建造入级船舶。
For plan approval, this application applies to a series of sister vessels; for survey during construction, this application applies to only 1 vessel.

下面签名者（申请方）确认知悉了本申请书中的重要提示和CCS最新颁布的《钢制海船建造与入级规范》中有关条款，包括但不限于，总则部分的所有条款的内容，并同意接受这些条款的约束。
The undersigned party (the Applicant) acknowledge all the articles in the Important Notice contained herein and the relevant terms in the latest edition of Rules and Regulations for the Construction and Classification of Seagoing Steel Ships of CCS, including, but not limited to, the General Part of above mentioned rules and regulations have been reviewed, and agree to be governed by those articles and terms.
我们保证按规定支付入级检验费：包括检验费和验船师为执行检验工作所必需的差旅费和其它开支。即使此项检验未能由船级社完成，我们也同意根据已进行的工作按比例向中国船级社支付相应的费用。
We agree to pay the fee for the specified service and, in addition, any traveling and other expenses incurred by the surveyor related to the services. In the event of this project not being completed under the Society's services, we also agree to pay on a proportion rata basis for the services provided.

重要提示：
1. 本申请一经申请方签署和盖章并被中国船级社（以下简称“本社”）受理，即构成本社与申请方之间的合同的证明。除非另有约定，下列条款和本社最新颁布的《钢质海船建造与入级规范》(以下简称“规范”)中的有关条款，包括但不限于规范中总则部分的条款，构成本社与申请方之间合同的内容,并调整与本申请有关的一切服务。如下述条款与本社规范中的条款规定有不一致的内容，以下列条款为准。
2.申请方保证在本申请表中所填写的内容和所提供的图纸、资料均属实。如查有不实之处，申请方应负法律责任，承担由此造成的一切后果。
3.如申请检验的船舶是转级船舶，本社有权从原船级社获得所有有关的资料文件。
4. 责任限定
4.1 本社按照合同提供服务，在任何情况下本社均不对与本社无直接合同关系方的任何损失承担责任。
4.2 本社仅仅对由于自身疏忽行为而直接造成的损失或损害承担责任，在任何情况下本社均不对间接损失或随后引发附加损失或损害承担责任。
4.3 如经证明合同关系方所遭受的损失或损害是由于本社或本社雇员、代理人或本社其他代表方的疏忽行为造成的，本社将承担责任，并将支付赔偿。但此赔偿的数额不超过该项服务收费的2 倍，且最大不超过人民币80 万元。但如该损失或损害系由如下行为所造成本社将不承担任何责任：
（1）本社雇员超越其受雇权限的行为；
（2）本社的代理人或其他代表方超越本社对其书面授权范围的行为。
4.4 对本社承担责任的损失或损害的索赔，必须以书面形式在损害最初被发现或损失形成的6 个
月内提出，否则将被视为彻底放弃索赔权。
5. 法律适用和仲裁
中华人民共和国法律适用。除本社另有约定外，所有由本社提供的服务引起的，或与本社提供的服务有关的任何争议，均应提交中国海事仲裁委员会，按照申请仲裁时该会现行有效的仲裁规则进行仲裁。仲裁裁决是终局的，对双方均有约束力。
Important Notice：
1. Once this Application with the Applicant’s signature and stamp is processed by China Classification Society (hereinafter referred to as “this Society”), This Application shall be deemed the evidence of contract between this Society and the Applicant. Unless otherwise agreed with this Society, the articles below and the relevant terms in the latest edition of Rules and Regulations for the Construction and Classification of Seagoing Steel Ships of this Society (hereinafter referred to as “the Rules”), including, but not limited to, the General Part of the Rules, constitute the contract between this Society and the Applicant, which governs all services provided in connection with this Application. In the event of any conflict or inconsistency between the articles below and the relevant terms in the Rules, the articles below shall prevail to the extent necessary to resolve such conflict or inconsistency.
2. The Applicant guarantees that all contents declared in this Application and all materials submitted are true. If any false data are found, the Applicant shall assume legal responsibility, and hold responsibility for all the serious consequences arising therefore.
