Minimum Requirements of HSSE competence PERSONNEL

HSSE人员的最低能力要求
1. Education background: Technical school / high school or above qualification
2.Working experience- Have at least 2 years in the construction industry and 1 year HSSE management working experience.
3. competence:
- understand national and local law & regulations relevant to the contract work
- can assess the risk level in the work, provide solution and develop action plan to reduce the high risk
- monitor site HSSE performance and can find incompliance on site, including worker behavior, PPE, process, i.e
- diagnose the gap for company and site HSSE vs target, propose solution and persuade management team to accept it , and implement it
- can conduct training to staff on site as per competence matrix
- audit experience is preferred

Phase 2: HSSE requirement for contractor (1 year after contract awarded)

1. Contractor HSE Management System

CONTRACTOR shall have in place an HSE-MS that includes at a minimum the following elements:

CONTRACTOR senior management shall provide strong, visible leadership and commitment to HSE management by:

· Allocating adequate resources for HSE management, including project and site HSE staffs, qualified equipment, facilities & PPE
· Attending HSE relevant trainings and meetings
· Conducting site HSE visits personally

1.2 Policy & Strategic Objectives

Shall have a written HSE policy addressing the same objectives as that of the SHELL’s HSE policy. The HSE policy shall be signed by CONTRACTOR’s senior management, and widely disseminated and understood among CONTRACTOR’s personnel.

Shell China HSE Policy & Commitment, Group Golden Rules are included in Appendix 1.

SHELL ENGINEER or his designated ENGINEERs have the right to prohibit commencement of WORKs or to stop any WORKs in progress if the equipment, machinery, personnel or work conditions are considered to be unsafe or not to be in compliance with any applicable HSE rules, regulations and procedures.

Stoppage of the WORK shall be at CONTRACTOR’s expense until CONTRACTOR has satisfactorily rectified such unsafe acts and condition. In the event of serious or repeated infringements, SHELL may terminate the contract without compensation.
有一套书面的HSSE政策，并与壳牌的HSE政策一致。HSE政策要有承包商的高级管理层签发，并在承包商的员工范围内广泛发布，确保所有的员工都理解并执行。

1.3 Organization, Responsibilities, Resources, Competence, Standards and Documents

Organisation & Responsibilities

· HSE is line responsibility at all levels of CONTRACTOR’s organisation, and it shall not be delegated to HSE Advisors/supervisors.
· HSE roles and responsibilities of all personnel should be clearly specified, and understood within all organisation
· CONTRACTOR management shall have HSE-dedicated meetings/committees to review and communicate HSE issues, and correct all the issues by following the recommendations in a timely manner according to the degree of concern.
· HSE是承包商组织架构中直线经理的责任，不能授权给HSE顾问、主管。
· 要清楚地规定所有人员的HSE角色和职责，讨论并沟通HSE事宜，按照问题的重要程度支持采取措施来解决所存在的问题。

Resources & Competence

· Ensure that competent personnel are fully trained and certified to carry out the task of operating all machinery, equipment, and tools.
· Provide an adequate level of technical and HSE related training conducted by a recognized training institution for all relevant personnel in connection with the WORK.
· All CONTRACTOR supervisory personnel including KEY PERSONNEL shall attend relevant HSE training and be competent in hazard identification.
· Ensure that all CONTRACTOR personnel engaged in the WORKs are healthy and fit, and posses the experience and skill required to perform the WORKs.

· It is required that CONTRACTORs give their feedback in written as well for replying, changing proposal, or reporting if the SHELL communicate with CONTRACTORs in written on any HSE issues.

Standards and Documents

Contractor shall have, not limited to, the following procedures in place:

· Detailed HSE requirement for specific construction work
· Accident reporting procedure, including accident definition and category criteria
· Emergency response procedure, including medical emergency/evacuation plan
· Permit to Work procedure
· Environmental & Waste management procedure
· Minimum standard for site camps & office
· Site HSE general rules
· Incentive scheme and discipline procedure

1.4 Hazards Effects Management Process
危害及影响管理程序

Shall conduct assessments of all WORK within its area of responsibility to:

· Identify HSE hazards and effects,
· Assess the significance of these HSE effects, and analyse the adequacy of existing controls,
· Controls & Recovery - develop/implement mitigation and recovery controls to minimise these effects to the extent practicable.

The mitigation and recovery controls shall be incorporated into the CONTRACT HSE Plan.

1.5 Planning

Develop a CONTRACT HSE plan to demonstrate that CONTRACTOR’s HSE MS is to be well implemented.

1.6 Implementation & Monitoring

· Report to SHELL immediately of the occurrence of any incident / accident including near misses according to SHELL accident reporting format.
· Maintain accurate incident and injury reports (including formal investigation) and shall comply with SHELL’s requests for information relating to accidents, injury or illnesses.
· Immediately report any knowledge of criminal activity or offence to the SHELL.
· Establish and document HSE monitoring programmes and procedures for data collection, interpretation and maintenance.
· Compile HSE performance reports to SHELL as per SHELL’s requirement.
· 所有的事故包括险肇事故要按照壳牌的事故报告模版在发生后马上汇报给壳牌
· 进行准确的事故和伤害报告（包括正式的调查），并与壳牌的关于事故，伤害，疾病的分类要求一致

1.7 Audit
审计
Develop and implement an HSE audit programme and audit follow-up process for SHELL approval.
制订并执行HSE审计计划以及审计跟进程序，并需得到壳牌批准。

SHELL reserves the right at any time to independently audit/review/inspect CONTRACTOR’s facilities, equipment, procedures and HSE management systems against the requirements of the CONTRACT. CONTRACTOR shall fully co-operate with SHELL in such audits and reviews and shall implement recommendations in a timely manner, at its own cost.
壳牌保留随时对承包商的设施，设备，程序和HSE管理系统按照合同要求进行审计，回顾，检查。承包商在审计或回顾过程中要与壳牌全面合作，并及时地执行改进计划，成本自负。

1.8 Management Review

· CONTRACTOR senior management shall review annually its performance against its HSE objectives and targets.
· CONTRACTOR line management shall review and update HSE management controls, as appropriate.

2. Contract HSE Plans
合同HSE方案
The CONTRACTOR shall prepare and submit for SHELL review an HSE plan which covers all WORK under the CONTRACT and, when relevant, legal required HSE documents. This shall be submitted to SHELL prior to the EFFECTIVE DATE of the CONTRACT or when physical work commences, whichever is the earlier.
承包商要准备并提交给壳牌HSE方案，内容包括合同项下的所有工作，包括法律要求所需要的文件，供壳牌审批。这要在合同生效日或现场工作开始前提交给壳牌，以两者中靠前的时间为准。

2.1 Hazard Assessment
危害评估
CONTRACTOR shall employs best practice methods in identifying, assessing and controlling hazards associated with the WORK prior to or in conjunction with its development of the CONTRACT HSE Plan, especially for the activities of:
承包商在制订合同HSE方案前或同时要采取合适的方式来识别，评估，控制工作中所涉及到的危害，尤其是以下活动：

· Heat work in high risk area
· Entry to confined space (depth >=1.5m)
· Flammable gas burning
· Live electric work
· Asbestos work
· Excavate work >=1.5m
· Work involved to explosive
· Work at height (>=1.8m)
· Other high risk work defined by SHELL
· 危险区域的高温作业

These methods shall be documented. Priority should be given to reduce probability, but consequence reducing measures shall be defined in case of failing controls.

It shall also evaluate the adequacy of existing controls and identify and recommend additional methods for managing / minimising the HSE risks.

2.2 CONTRACT HSE Plan

After conducting a detailed hazard assessment of the WORK, the CONTRACTOR shall develop a CONTRACT HSE Plan in which, as minimum, describes the followings:

· Full understanding of the requirements in the HSE Specifications
· Contractor HSE targets and Key Performance Indicators, i.e.
- Lost Time Injury Frequency (LTIF)
- Total Recordable Case Frequency (TRCF)
- NM/PI report
· Identified all HSE-related activities to be performed during the CONTRACT, including but not limited to the identification of action parties and specification of completion dates
· Identified and assessed the hazards/risks anticipated during the execution of the CONTRACT
· How it will manage the identified hazards/activities anticipated, and mitigate risks associated with the Scope of Work
· How compliance with SHELL’s requirements will be measured and achieved
· HSE improvement initiatives describing any current or proposed HSE improvement initiatives or programs relevant to the WORK
· Numbers of HSE Advisors to be employed on the CONTRACT, including but not limited to a description of their roles, responsibilities and deployment
· A description of CONTRACTOR’s HSE audit / inspection / monitoring plans / programs for the CONTRACT, including a Schedule of HSE management activities to be undertaken during mobilisation, execution of the WORK and demobilisation (e.g. HSE meeting structure and site visits/inspections)

The CONTRACT HSE Plan shall obtain SHELL’s approval prior to the commencement of WORK. The approved CONTRACT HSE Plan shall become part of the CONTRACT documents.

CONTRACTOR shall obtain formal approval from SHELL for any proposed deviations or amendments to the CONTRACT HSE Plan.

合同HSE方案要在工作开始前得到壳牌的批准。批准的合同HSE方案将成为合同文件的一部分。

3. HSE Meetings

CONTRACTOR shall hold regular HSE meetings at different levels of the organization on the following basis, at a minimum:
· SITE management meeting Weekly
· Toolbox meeting (HSE talks) at site Daily -before starting work
· Project management meeting Project close out

Toolbox meetings shall be held by each work team at individual work SITES before commencement of the day’s activities or prior to each shift. They shall concentrate on the risks of the WORK being undertaken and the precautions necessary to ensure a safe, healthy and environmentally acceptable working day. CONTRACTOR shall maintain records of attendance and submit to SHELL as and when required.

The project management meeting shall be chaired by the CONTRACTOR project manager, and attended by all project management members, and site representatives. Minutes of meeting shall be distributed to the SHELL by the end of the project.

承包商要在不同层面定期召开HSE会议，至少要按照以下频率：
· 现场管理会 每周一次
· 现场班前会 每天开始工作前
· 项目管理会 项目结束时

班前会要由每个工作小组在各自的现场在开始每天的工作前或换班前进行。内容集中在当天将要进行的工作的所涉及到的风险，预防措施确保安全、健康、环保方面达到可就接受的水平。承包商保存参会人员的记录并在壳牌需要时能提供。
项目管理会议将由承包商项目经理主持，所有的项目管理人员和现场代表参加。会议纪要要在项目结束时发给壳牌。

4. Competence of Contractor Personnel
承包商人员能力

The CONTRACTOR shall ensure that only competent CONTRACTOR PERSONNEL shall be provided for the performance of the WORK.

The CONTRACTOR shall avoid turnover of CONTRACTOR PERSONNEL where possible and shall obtain SHELL’s written approval prior to replacing any member of KEY CONTRACTOR PERSONNEL and/or any member of CONTRACTOR PERSONNEL who perform work involving a specific risk.
